Bruising and Swelling After a Leg Injury: When to See a Doctor - Family Medicine

Calf contusion?

Hello doctor, about two weeks ago I accidentally fell down the stairs.
I have bruises on the front of both calves and my knees.
The bruising on my right calf and both knees is slowly recovering, but the impact on my left calf may have been more severe.
During the first week, there was a large swelling in the injured area, and walking was somewhat painful, although the bruising was not very noticeable.
Now the swelling has gradually decreased, but I have developed patches of bruising on the inner side of my ankle and nearby areas for no apparent reason, which only hurts when pressed.
Should I see a doctor for this condition? If so, which specialty should I consult?

After a leg injury, such as falling down stairs, it is common to experience bruising and swelling, particularly in the areas directly impacted. In your case, it sounds like you have sustained some soft tissue damage, which is often characterized by bruising (ecchymosis) and swelling (edema). The fact that you initially had a significant swelling and pain in your left calf, which has since improved, is a positive sign. However, the emergence of new bruising around the ankle area and persistent pain upon pressure warrants further evaluation.

Bruising occurs when small blood vessels (capillaries) are damaged, allowing blood to leak into the surrounding tissues. This can happen due to trauma, such as a fall. The swelling you experienced is likely due to inflammation and fluid accumulation in response to the injury. It is also important to note that the body’s healing process can sometimes lead to the appearance of new bruises as blood is reabsorbed and redistributed in the area.

Given that you are experiencing new bruising and localized pain, it is advisable to consult a healthcare professional. While the initial injury may not have resulted in significant damage, the new symptoms could indicate a few possibilities:
1. Soft Tissue Injury: You may have sustained a contusion or strain that is now manifesting as bruising in the ankle area. This is common after an injury, as the body continues to react to the trauma.

2. Compartment Syndrome: Although less common, it’s important to be aware of this condition, which occurs when swelling within a closed muscle compartment increases pressure to the point that it compromises blood flow. Symptoms include severe pain, swelling, and sometimes numbness or tingling. If you experience increasing pain, numbness, or a change in color of the skin, seek immediate medical attention.

3. Fracture or Ligament Injury: While you mentioned that the bruising was not very pronounced, it’s possible that there could be a minor fracture or ligament injury that was not initially detected. If the pain persists or worsens, further imaging studies, such as X-rays or an MRI, may be warranted to rule out these possibilities.

In terms of which specialist to see, starting with an orthopedic doctor would be appropriate, as they specialize in musculoskeletal injuries. They can perform a thorough examination and may recommend imaging studies to assess the extent of your injury. If necessary, they may refer you to a physical therapist for rehabilitation to strengthen the affected area and improve mobility.

In summary, while some bruising and swelling are normal after an injury, the development of new symptoms, particularly if they are accompanied by pain, should not be ignored. It is best to consult a healthcare professional to ensure that there are no underlying issues that require treatment. Early intervention can help prevent complications and facilitate a quicker recovery, allowing you to return to your normal activities without long-term effects.

If you can walk, there's no need to worry, because even a slight hairline fracture would cause enough pain to prevent walking. If it's just a bruise, it should gradually stop hurting in about two weeks.

Bruises may occur due to bleeding from muscle impact. After an injury, you can apply heat to alleviate swelling after three days. If there is pain, over-the-counter analgesics and anti-inflammatory medications can be taken for treatment.
