Persistent Bruising After Injury: When to Seek Medical Help - Family Medicine

Stagnant blood?

Hello doctor, I was in a car accident at the end of September, and my left calf was crushed by a motorcycle, resulting in bruising.
The bruising has improved but has not fully healed, and there is still a large area of light-colored bruising on my leg.
When I apply a heating pad, it causes redness and swelling.
A massage therapist mentioned that my left ankle ligament injury from before has never fully healed (my walking posture is off, leading to complications), causing blockages in the meridians, which makes it difficult for the bruising to resolve.
Given that it has been over two months, which specialty should I see for treatment? If the bruising does not dissipate, will it have any consequences? Thank you.

Hello: Your ongoing bruising raises concerns about the possibility of fractures, ligament tears, or tendon ruptures.
However, if you are still able to walk normally, the likelihood of a fracture may be low.
It is more probable that you are experiencing a ligament or tendon tear.
I recommend visiting an orthopedic specialist for an evaluation.

Persistent bruising after an injury can be concerning, especially when it lasts for an extended period, as in your case. Given that you experienced a significant bruise on your left calf from a motorcycle accident in late September, and it has not fully resolved after two months, it is important to consider a few factors regarding your condition and the appropriate steps to take.

Firstly, bruising occurs when small blood vessels (capillaries) break under the skin, allowing blood to leak into the surrounding tissue. This can happen due to trauma, such as a car accident. Typically, bruises change color as they heal, transitioning from red to purple, blue, green, and finally yellow before fading away completely. However, if a bruise persists for an extended period, it may indicate underlying issues that need to be addressed.

In your situation, the fact that the bruise has not fully resolved and that you are experiencing symptoms such as redness and swelling when applying heat suggests that there may be ongoing inflammation or possibly a hematoma (a localized collection of blood outside of blood vessels). The advice from the massage therapist regarding previous ankle ligament injuries and the potential for blocked circulation is also worth considering. Chronic issues with the ankle could lead to altered biomechanics, which might affect how blood flows and how quickly bruises heal.

Given these factors, it is advisable to seek medical attention. You should consider visiting a healthcare provider who specializes in musculoskeletal injuries, such as an orthopedic doctor or a physical medicine and rehabilitation specialist (physiatrist). They can perform a thorough examination, possibly including imaging studies like an ultrasound or MRI, to assess the extent of the injury and determine if there are any complications such as a hematoma or other soft tissue injuries.

It is also important to monitor for any signs of complications. If you experience increased pain, swelling, warmth, or redness in the area, or if you develop fever or chills, these could be signs of infection or other serious conditions that require immediate medical attention.

In terms of treatment, if the bruise is indeed a hematoma or if there is significant swelling, your doctor may recommend rest, ice, compression, and elevation (the RICE method) to help reduce swelling and promote healing. Physical therapy may also be beneficial to address any lingering issues related to your previous ankle injury and to improve circulation and mobility in your leg.

In summary, while bruising can be a normal response to injury, persistent bruising that does not resolve after two months warrants further evaluation. Seeking care from an orthopedic specialist or a physiatrist will help ensure that you receive an accurate diagnosis and appropriate treatment to facilitate healing and prevent any long-term complications. Your health and recovery are paramount, and addressing these concerns early can lead to better outcomes.
