Development of myasthenia gravis after receiving the AZ vaccine?

Hello Doctor: After receiving my first dose of the AZ vaccine in mid to late July 2021, I noticed that I frequently fell from late July to early August.
I initially thought it was due to not exercising for a long time, but after going to the gym for leg presses, my legs felt weak (I also noticed weakness when lifting weights, even with the lightest ones).
I didn't think much of it at the time, but I started to fall more often in stairwells or while getting dressed and drying my hair.
Therefore, in October, I consulted the neurology department at National Taiwan University Hospital, where I underwent electromyography and blood tests, but no disease was detected at that time.
I also visited an ophthalmologist regarding eye discomfort, who suggested it might be due to ingrown eyelashes (later discovered to be double vision caused by myasthenia gravis).
I then experienced chewing weakness and stiffness in my lower back, frequently falling and getting injured, with my condition continuing to worsen.
I subsequently sought care at the family medicine department of Tri-Service General Hospital and was referred to their neurology department.
After another round of electromyography, blood tests, and consultations, I was diagnosed with generalized myasthenia gravis unrelated to thymoma.
I am currently on medication, which alleviates symptoms, but I still feel severely limited in strength and stamina, and I occasionally still fall.
Given that the onset of symptoms coincided with the administration of my first dose of the AZ vaccine, and my doctor recommended switching to Moderna for the booster shot due to the higher incidence of adverse immune reactions with AZ, I would like to know if there are any reported cases of vaccine-related incidents like this.
Thank you very much!

The concerns regarding potential neurological issues following the administration of the AstraZeneca (AZ) vaccine, particularly in relation to Myasthenia Gravis (MG), are indeed significant and warrant careful consideration. Myasthenia Gravis is an autoimmune disorder characterized by weakness and rapid fatigue of voluntary muscles, which can manifest in various ways, including difficulty in swallowing, speaking, and controlling eye movements. The onset of symptoms can sometimes be insidious, as seen in your case.

Your experience following the AZ vaccination, where you began to notice symptoms such as frequent falls, muscle weakness, and ocular issues, aligns with the types of symptoms that can be associated with MG. It is important to note that while there have been reports of neurological complications following COVID-19 vaccinations, including the AZ vaccine, these occurrences are relatively rare. The relationship between vaccination and the onset of autoimmune conditions like MG is still under investigation, and current evidence does not definitively establish a causal link.

In your situation, the timeline of symptom onset shortly after vaccination raises a valid question about whether the vaccine could have triggered or exacerbated an underlying autoimmune process. The fact that you underwent electromyography (EMG) and blood tests, which did not initially reveal MG, suggests that the diagnosis may not have been straightforward. MG can sometimes be challenging to diagnose, especially in its early stages or when it presents atypically.

It is also noteworthy that the medical community has observed cases where vaccination may act as a trigger for autoimmune conditions in predisposed individuals. This phenomenon is not unique to the AZ vaccine; it has been documented with other vaccines as well. However, the benefits of vaccination in preventing severe illness from COVID-19 generally outweigh the risks of potential adverse effects, including autoimmune reactions.

Given your ongoing symptoms and the confirmation of generalized Myasthenia Gravis, it is crucial to continue working closely with your healthcare providers. Treatment for MG typically involves medications such as anticholinesterase agents, immunosuppressants, or even plasmapheresis in more severe cases. Monitoring your symptoms and adjusting treatment as necessary is essential to managing this condition effectively.

As for the recommendation to switch to the Moderna vaccine for your booster shot, this is a common practice in individuals who have experienced adverse reactions to a specific vaccine. The Moderna vaccine, while still a messenger RNA (mRNA) vaccine, has a different formulation and may elicit a different immune response. It is important to discuss this option with your healthcare provider, who can assess your individual risk factors and medical history to make the best recommendation for your booster vaccination.

In summary, while there are documented cases of neurological issues following the AZ vaccine, including MG, the evidence remains inconclusive regarding a direct causal relationship. Your symptoms should be managed with appropriate medical care, and any vaccination decisions should be made collaboratively with your healthcare team, considering both the risks and benefits involved. It is essential to remain vigilant about your health and report any new or worsening symptoms to your doctor promptly.
