Nasal issues?
Hello Dr.
Hu: I have been troubled by nasal issues for many years.
I have allergic rhinitis, a deviated septum, and asthma.
Sometimes when I have an episode, just touching my nose causes pain, and it feels like there are wounds inside my nose.
I also experience headaches, weakness, and chills (even on hot days).
Sometimes when I eat, I can't taste anything, feeling as if I have lost my sense of taste.
Currently, I am using antihistamine nasal sprays and oral medications.
Initially, my symptoms improved during the first week of use, but afterward, it seems like the medication is no longer effective.
Sometimes at night, I experience difficulty breathing.
I would like to know if these are complications of allergic rhinitis, and if there are any effective medications or injections to alleviate allergic rhinitis.
Thank you!
sandy, 10~19 year old female. Ask Date: 2003/10/29
Dr. Hu Kexin reply Otolaryngology
Hello: It seems that your allergy symptoms are quite severe.
Allergies can indeed cause significant disruption in daily life and affect quality of life.
The symptoms you described are also consistent with complications of allergic rhinitis.
The treatment for allergic rhinitis primarily involves medication (oral and nasal sprays), but if medication is not effective, surgical options may be considered to improve or alleviate symptoms.
I recommend that you manage your allergic rhinitis effectively to prevent complications such as sinusitis or nasal polyps.
Best wishes! Sincerely, Dr.
Hsu, Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare, October 31, 2003.

More Info
Allergic rhinitis is a common condition that can significantly impact your quality of life, especially when it is accompanied by other issues such as a deviated septum and asthma. The symptoms you describe—pain upon touching the nose, headaches, fatigue, and even loss of taste—are indeed consistent with complications related to allergic rhinitis.
Understanding Allergic Rhinitis and Its Complications
Allergic rhinitis occurs when your immune system overreacts to allergens in the air, such as pollen, dust mites, or pet dander. This reaction leads to inflammation of the nasal passages, resulting in symptoms like nasal congestion, sneezing, itching, and runny nose. In your case, the presence of a deviated septum may exacerbate these symptoms by obstructing airflow and drainage in the nasal passages, leading to further complications such as sinusitis or nasal polyps.
The pain you feel in your nose could be due to inflammation and irritation of the nasal mucosa. This inflammation can also lead to headaches, as the pressure builds up in the sinuses. The sensation of not being able to taste food properly is often linked to nasal congestion, which can block the olfactory receptors responsible for smell, thus affecting taste.
Treatment Options for Allergic Rhinitis
1. Medications:
- Antihistamines: These are often the first line of treatment for allergic rhinitis. They help reduce sneezing, itching, and runny nose. However, as you noted, their effectiveness can diminish over time. If you find that your current antihistamine is no longer effective, it may be worth discussing with your doctor about trying a different one or combining medications.
- Nasal Corticosteroids: These are highly effective for reducing inflammation in the nasal passages. They can help alleviate nasal congestion and other symptoms. If you haven't tried a nasal corticosteroid yet, it might be beneficial to discuss this option with your healthcare provider.
- Decongestants: These can provide temporary relief from nasal congestion but should not be used for extended periods due to the risk of rebound congestion.
2. Allergy Shots (Immunotherapy): If your allergic rhinitis is severe and not well-controlled with medications, your doctor may recommend allergy shots. This treatment involves receiving regular injections of allergens over time, which can help your body build tolerance and reduce symptoms.
3. Lifestyle Modifications:
- Avoiding Triggers: Identifying and avoiding allergens that trigger your symptoms is crucial. This may involve using air purifiers, keeping windows closed during high pollen seasons, and regularly cleaning your living space.
- Nasal Irrigation: Using saline nasal sprays or a neti pot can help clear allergens and mucus from your nasal passages, providing relief from congestion.
4. Surgery: In cases where structural issues, such as a deviated septum, significantly contribute to your symptoms, surgical intervention may be necessary. A septoplasty can correct the deviation, improving airflow and potentially reducing the severity of your allergic rhinitis symptoms.
Conclusion
It is essential to work closely with an ear, nose, and throat (ENT) specialist or an allergist to develop a comprehensive treatment plan tailored to your specific needs. Given the complexity of your symptoms and the presence of multiple conditions (allergic rhinitis, deviated septum, asthma), a multidisciplinary approach may be beneficial. Regular follow-ups and adjustments to your treatment plan can help manage your symptoms effectively and improve your overall quality of life.
