Regarding emergency contraceptive pills and the COVID-19 vaccine?

Hello Doctor, I am currently taking the oral contraceptive pill - Aivee (24+4 formulation), and I have some questions:
1.
I have a cold, and based on my previous research, I found that some medications that interact with oral contraceptives include Acetaminophen and Ascorbic acid (Vitamin C).
If I take cold medication and the contraceptive pill with a 2-hour interval, will it still reduce the effectiveness of the contraceptive? Should I use additional contraceptive measures?
2.
How many days should I wait after taking the oral contraceptive pill before receiving the COVID-19 vaccine? Or is it still fine to continue taking it without any concerns? Thank you!

TING, 30~39 year old female. Ask Date: 2022/12/19

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, it generally does not have any impact, so you can continue taking the contraceptive pills as usual.
Wishing you good health.

When considering the interaction between birth control pills, specifically the combined oral contraceptive (COC) like "Aiwei" (24+4 regimen), and COVID-19 vaccines, it is essential to understand both the pharmacological aspects and the current medical guidelines.

1. Interaction with Cold Medications: You mentioned concerns about taking acetaminophen (Tylenol) and ascorbic acid (Vitamin C) while on birth control pills. Generally, acetaminophen does not have a known interaction with hormonal contraceptives and should not affect their efficacy. Ascorbic acid, while it can influence the metabolism of certain medications, is not typically associated with a significant impact on the effectiveness of birth control pills when taken in normal dietary amounts. The key point is that taking these medications two hours apart should not reduce the contraceptive effect of your birth control pills. However, if you are taking any prescription cold medications, it is always best to consult with your healthcare provider to ensure there are no specific interactions.

2. Timing of COVID-19 Vaccination: Regarding the timing of COVID-19 vaccination in relation to taking birth control pills, there is no requirement to stop or delay your contraceptive method before receiving the vaccine. You can continue taking your birth control pills as usual while getting vaccinated. The vaccines currently authorized for emergency use have not shown any adverse interactions with hormonal contraceptives. Therefore, you do not need to take additional contraceptive measures solely because you are getting vaccinated against COVID-19.

3. General Recommendations: It is crucial to maintain your regular contraceptive regimen even while experiencing mild illnesses like a cold. If you have any concerns about the effectiveness of your birth control due to illness or medication, consider using a backup method, such as condoms, until you feel confident in your contraceptive protection again.
4. Consulting Healthcare Providers: If you have specific health concerns or conditions that may complicate your use of birth control or vaccination, it is always advisable to discuss these with your healthcare provider. They can provide personalized advice based on your medical history and current health status.

In summary, taking acetaminophen and Vitamin C should not affect the efficacy of your birth control pills, and you can receive the COVID-19 vaccine without needing to alter your contraceptive regimen. Always consult with your healthcare provider for tailored advice and to address any specific health concerns you may have.
