Itchy penis?

Dear Doctor,
One day, it seemed like I didn't dry myself properly after urinating, as I often experience this issue.
A few hours later, in the evening, I noticed that my foreskin was slightly red and swollen, and it started to itch.
I realized something was wrong and bought an antifungal ointment to apply around the itchy area of my foreskin, which provided some relief.
However, after taking a shower the next day and not applying the ointment again, the itching returned.
This is a situation I've never experienced before.
In the past, I have had urinary tract infections, but this time I don't have any burning sensation in the urethra, so it shouldn't be a urinary tract infection, right? It's a holiday weekend, and there are no clinics open.
I'm also worried that this could be a sexually transmitted infection, but I haven't been with anyone recently.
Dr.
Liu, please help me with this issue.
Thank you!

It seems quite complicated and may be bothering you.
The urethra, glans, and foreskin often experience some redness, swelling, and itching.
If there has been no sexual contact, it is likely not a sexually transmitted infection; it may just be a common fungal infection or dermatitis.
I recommend visiting a hospital to consult a urologist for examination and treatment.

Itchy symptoms in the genital area, particularly after urination, can be concerning and may indicate an underlying issue that requires attention. Based on your description, it seems you are experiencing symptoms that could be related to several conditions, including infections, irritations, or even skin conditions.

Firstly, the sensation of not being able to "shake dry" after urination can lead to moisture accumulation, which may create an environment conducive to irritation or infection. This moisture can cause the skin to become macerated, leading to redness and itching, as you have described. The fact that you noticed redness and swelling of the foreskin (prepuce) and glans (head of the penis) suggests that there may be some form of inflammation or infection present.

One common cause of these symptoms is balanitis, which is inflammation of the glans. This can occur due to poor hygiene, irritants (such as soaps or lotions), or infections (bacterial or fungal). Given that you mentioned using an antifungal ointment, it is possible that a fungal infection could be contributing to your symptoms, especially if the area remained moist.

Another possibility is a sexually transmitted infection (STI), even if you believe you have not engaged in risky sexual behavior. Some STIs can present with itching and irritation without the typical symptoms of burning or discharge. However, if you have not had any new sexual partners or unprotected encounters, the likelihood of an STI may be lower.

In terms of treatment, it is essential to maintain good hygiene. Ensure that the genital area is kept clean and dry. You might consider using a mild, unscented soap and thoroughly drying the area after bathing. If you suspect that the ointment you are using is not effective, it may be beneficial to consult a healthcare provider for a more targeted treatment. They may recommend a different antifungal or even a topical steroid if inflammation is significant.

If your symptoms persist or worsen, it is crucial to seek medical attention. A healthcare provider can perform a physical examination and may recommend tests, such as a urinalysis or swabs, to determine the exact cause of your symptoms. They can also rule out any serious conditions, including STIs or other infections.

In summary, while your symptoms may not necessarily indicate a severe condition, they warrant further evaluation, especially if they do not resolve with over-the-counter treatments. Maintaining good hygiene, avoiding irritants, and consulting a healthcare professional if symptoms persist are essential steps in addressing your concerns.
