Blackheads
Hello Dr.
Cheng: I have been struggling with severe blackheads on my nose and chin since puberty.
Generally, I have never been able to completely clear them out.
Whether I have facials done or try to extract them myself using pore strips or a comedone extractor, they seem to reappear within a couple of days.
My pores are getting larger, and often, when I gently scrape my skin with my fingers, yellowish-white sebum (which traps dirt from the air) comes out, especially noticeable in the summer.
Occasionally, my nose also peels, and I'm unsure if this is related to the acne treatment products I have been using.
Additionally, due to the blackhead issue, the pores on my nose and the sides of my cheeks are quite large and prominent, making my nose look like a large, dark strawberry.
Therefore, I would like to ask if the dermatology department can help me address my blackhead and enlarged pore issues, as facial treatments are really painful and troublesome.
I truly want to find a permanent solution to these problems.

To clear acne, salicylic acid, retinoids, alpha hydroxy acids (AHAs), and azelaic acid can be used as exfoliating agents to gradually soften and remove comedones.
While pore strips may provide immediate satisfaction in extracting blackheads, their excessive and aggressive use is not advisable as it can damage the surrounding healthy skin and hair follicles.
Proper cleansing, exfoliation, and avoiding overly oily skincare products are essential to prevent pore blockage.
For oily skin, in addition to maintaining facial cleanliness, it is helpful to keep tissues or oil-absorbing sheets on hand for quick blotting of excess oil.
Using a sebum-controlling serum can also make managing oil production easier.

More Info
Dealing with persistent blackheads and enlarged pores can be a frustrating experience, especially if you've been struggling with these issues since adolescence. It's important to understand that blackheads are a type of acne that occurs when hair follicles become clogged with oil, dead skin cells, and bacteria. Enlarged pores often accompany blackheads, as they can stretch due to the buildup of sebum and debris. Here are some effective solutions and treatments that can help you manage and potentially reduce these skin concerns.
1. Regular Cleansing Routine
Establishing a consistent skincare routine is crucial. Use a gentle cleanser twice daily to remove excess oil and impurities. Look for products containing salicylic acid or glycolic acid, as these ingredients can help exfoliate the skin and prevent clogged pores. Salicylic acid is particularly effective because it penetrates deep into the pores, helping to dissolve the debris that leads to blackheads.
2. Exfoliation
Regular exfoliation is key to preventing blackheads and minimizing the appearance of enlarged pores. You can use physical exfoliants (scrubs) or chemical exfoliants (like AHAs and BHAs). However, be cautious with physical scrubs, as they can irritate the skin if used too aggressively. Chemical exfoliants tend to be gentler and can provide more consistent results.
3. Topical Treatments
Consider incorporating topical treatments that contain retinoids, which are derivatives of vitamin A. Retinoids help to increase cell turnover, preventing the buildup of dead skin cells that can clog pores. They also promote collagen production, which can help improve skin texture and reduce the appearance of enlarged pores.
4. Clay Masks
Using clay masks once or twice a week can help absorb excess oil and draw out impurities from the skin. Clay masks are particularly beneficial for oily and acne-prone skin, as they can help minimize the appearance of blackheads and pores.
5. Professional Treatments
If at-home treatments are not yielding the desired results, consider seeking professional help. Dermatologists can offer various treatments, including:
- Chemical Peels: These treatments use stronger acids to exfoliate the skin more deeply, helping to clear out blackheads and improve skin texture.
- Microdermabrasion: This procedure involves exfoliating the skin with tiny crystals to remove dead skin cells and promote new cell growth.
- Extraction: A dermatologist can safely extract blackheads using specialized tools, which can provide immediate results.
- Laser Treatments: Certain laser therapies can help reduce oil production and improve skin texture, leading to smaller-looking pores.
6. Lifestyle Considerations
Your lifestyle can also impact your skin health. Ensure you are drinking plenty of water, eating a balanced diet rich in fruits and vegetables, and managing stress levels. Avoid touching your face frequently, as this can transfer oils and bacteria from your hands to your skin, exacerbating blackheads and enlarged pores.
7. Avoid Over-Treatment
While it may be tempting to try multiple products at once, over-treating your skin can lead to irritation and worsen your condition. Stick to a few effective products and give your skin time to adjust.
Conclusion
In summary, managing persistent blackheads and enlarged pores requires a combination of a consistent skincare routine, effective topical treatments, and possibly professional interventions. It's essential to be patient, as results may take time. If you're unsure about which products or treatments to use, consulting with a dermatologist can provide personalized recommendations tailored to your skin type and concerns. Remember, achieving clear skin is a journey, and with the right approach, you can significantly improve your skin's appearance and health.
