Epididymal pain

Recently, I have been experiencing frequent pain in the area below my right testicle, approximately 4 to 5 times an hour.
During these episodes, I often find myself touching that area, which seems to be the epididymis.
I would like to know if this could be epididymitis.

Zhang, 10~19 year old female. Ask Date: 2023/02/28

Dr. Li Xinyi reply Urology

Based on your description, it does seem like an issue with the epididymis.
It's important to go to the hospital for an examination rather than trying to press on it yourself, as that could lead to inflammation and complicate treatment.

More Info

Epididymitis is an inflammation of the epididymis, which is a coiled tube located at the back of the testicle that stores and carries sperm. Understanding the causes, symptoms, and treatment options for epididymitis is essential, especially if you are experiencing pain in the area beneath the testicle, which you suspect may be related to the epididymis.

Causes of Epididymitis
Epididymitis can be caused by various factors, including:
1. Infections: The most common cause of epididymitis is a bacterial infection. In sexually active men, sexually transmitted infections (STIs) such as chlamydia and gonorrhea are frequent culprits. In older men, urinary tract infections (UTIs) or bacteria from the prostate can lead to epididymitis.

2. Non-infectious causes: Other factors can also lead to inflammation of the epididymis. These include trauma to the area, certain medications, or even a reaction to a medical procedure.

3. Other medical conditions: Conditions such as testicular torsion or a hernia can sometimes mimic the symptoms of epididymitis or contribute to its development.

Symptoms of Epididymitis
The symptoms of epididymitis can vary but typically include:
- Pain and swelling: The most prominent symptom is pain in the affected testicle, which may radiate to the groin. The swelling may be accompanied by tenderness in the area.

- Redness and warmth: The skin over the affected area may appear red and feel warm to the touch.

- Discharge: In cases caused by STIs, there may be a discharge from the penis.

- Urinary symptoms: Some individuals may experience pain during urination, increased frequency of urination, or urgency.

- Fever: In some cases, fever may accompany the symptoms, indicating a more severe infection.

Diagnosis
To confirm a diagnosis of epididymitis, a healthcare provider will typically perform a physical examination and may order tests such as:
- Urinalysis: To check for signs of infection.

- Ultrasound: To rule out other conditions such as testicular torsion or tumors.

- Swabs: If an STI is suspected, swabs may be taken for testing.

Treatment Options
Treatment for epididymitis depends on the underlying cause:
1. Antibiotics: If the cause is bacterial, antibiotics are the primary treatment. The choice of antibiotic may depend on the suspected source of the infection (e.g., STIs vs. UTIs).

2. Pain relief: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate pain and reduce inflammation.

3. Supportive care: Resting and elevating the affected testicle can help reduce discomfort. Applying ice packs to the area may also provide relief.

4. Follow-up care: It’s essential to follow up with your healthcare provider to ensure that the infection is resolving and to address any complications that may arise.

When to Seek Medical Attention
If you are experiencing persistent pain, swelling, or any other concerning symptoms, it is crucial to seek medical attention promptly. Early diagnosis and treatment can help prevent complications such as abscess formation or chronic pain.

In summary, based on your description of pain in the area beneath the testicle, it is possible that you may be experiencing epididymitis. However, a proper evaluation by a healthcare professional is necessary to confirm the diagnosis and initiate appropriate treatment.
