I have been troubled by psychological issues for over ten years?
My first visit to a psychiatrist is a bit hazy now; I only remember feeling that my family favored my younger brother over me.
This started from fifth grade until the first year of junior high school, and I was diagnosed with childhood emotional disturbance with depressive tendencies.
My second visit was during my first year of high school due to a wrong subject selection and a tumultuous relationship with my girlfriend, which led to conflicts with my family.
I lost sight of my purpose and ended up seeing a psychiatrist again.
This continued for two years, and then I had another episode starting last November.
I became very anxious, waking up in the middle of the night feeling tense and experiencing dry heaving, unable to fall back asleep.
It felt like a moment where I lost all sense of life and death; both seemed acceptable to me.
Recently, I've started to seek out pain to confirm that I still exist.
I often cry inexplicably, and these symptoms began mildly but have intensified from the end of 2020 to now.
I don't know what's wrong with me or what to do.
I am currently taking medication regularly and attending follow-up appointments, but the medication seems to have lost its effectiveness.
I wonder if it's worth spending on healthcare for someone like me with a chronic mental illness.
I hope the doctor can provide me with some direction.
What kind of psychological issue am I facing?

Hello.
Individuals with chronic mental illnesses, like everyone else, deserve to have a happier and more fulfilling life.
Based on your description, you may be experiencing depressive symptoms and anxiety.
However, if you feel that the medication is not effective, you should discuss adjusting your medication with your current physician.
Additionally, for non-pharmacological approaches, you might consider seeking psychotherapy.

It sounds like you are going through a very challenging time, and I want to acknowledge the pain and confusion you are experiencing. Chronic mental health issues can feel overwhelming, especially when they have persisted for years and seem to escalate over time. Based on your description, it appears that you have been dealing with a range of emotional difficulties, including anxiety, depression, and feelings of worthlessness stemming from your childhood experiences and family dynamics.
From a clinical perspective, your history of being diagnosed with childhood emotional disturbances and depressive tendencies suggests that you may be experiencing a form of chronic depression or anxiety disorder. The symptoms you describe, such as intense anxiety, panic attacks during sleep, and feelings of despair, are indicative of generalized anxiety disorder (GAD) or possibly major depressive disorder (MDD). The fact that you have had multiple episodes of seeking help and have been on medication indicates that this is a persistent issue that requires ongoing management.
It's important to understand that mental health conditions are complex and can be influenced by a variety of factors, including genetics, environment, and personal experiences. Your feelings of being unloved or less favored in your family, particularly in relation to your brother, may have contributed to your emotional struggles. These feelings can lead to low self-esteem and a distorted self-image, which can exacerbate anxiety and depressive symptoms.
You mentioned that you have been on medication but feel that it is no longer effective. This is not uncommon; sometimes, medications can lose their efficacy over time, or the initial diagnosis may not fully encompass the breadth of your experiences. It is crucial to communicate openly with your psychiatrist or mental health provider about your current symptoms and concerns regarding your medication. They may suggest adjusting your dosage, switching to a different medication, or incorporating therapy into your treatment plan.
Therapy can be a valuable resource for individuals dealing with chronic mental health issues. Cognitive-behavioral therapy (CBT) is particularly effective for anxiety and depression, as it helps individuals identify and challenge negative thought patterns and develop healthier coping strategies. Additionally, exploring underlying issues related to your family dynamics and self-worth in a therapeutic setting can provide insight and facilitate healing.
You also mentioned a desire to feel pain to confirm your existence, which is a concerning sign of emotional distress. It is essential to address these feelings with a mental health professional, as they can help you navigate these thoughts and feelings safely. Engaging in activities that promote self-care, such as exercise, mindfulness, and connecting with supportive friends or family members, can also be beneficial in managing your symptoms.
In summary, your journey with chronic mental health issues is significant, and it is essential to seek help and support. You are not alone in this struggle, and there are effective treatments available. Please consider reaching out to a mental health professional who can provide you with the guidance and support you need to navigate this challenging time. Remember, it is okay to ask for help, and taking that step can lead to a path of healing and recovery.
