Itching of the foreskin after urination?

Hello, doctor.
I would like to ask about a condition I've been experiencing for the past month.
After urination, the surface of my foreskin feels itchy, similar to being bitten by a mosquito, and it is accompanied by a stinging sensation.
This lasts for about 1 to 3 minutes, after which I feel no further discomfort.
However, this sensation recurs after each urination.
When I retract the foreskin, I notice some white substance on the underside of the skin, and touching it triggers that post-urination itchiness.
Could you please explain what this condition might be? Thank you.

W, 40~49 year old female. Ask Date: 2023/05/24

Hello: It could be a case of balanitis, which may cause a white discharge.
It's important to keep the area clean and dry, rinse with clean water, and dry thoroughly.
If there is diabetes, it should be managed.
If it does not improve in a few days, seek medical attention.
Wishing you good health.

Post-urination itching and discomfort under the foreskin can be a distressing issue, and it is important to understand the potential causes and solutions. Based on the symptoms you described—itching and a stinging sensation after urination, along with the presence of white substances under the foreskin—there are several possible explanations.

1. Balanitis: This is an inflammation of the glans (the head of the penis) and can occur due to various reasons, including poor hygiene, infections, or irritants. The white substance you noticed could be smegma, a combination of dead skin cells, oils, and moisture that can accumulate under the foreskin. If not cleaned regularly, it can lead to irritation and inflammation, causing itching and discomfort.

2. Yeast Infection: A yeast infection, particularly caused by Candida species, can lead to symptoms like itching, burning, and a white discharge. This type of infection is more common in individuals with diabetes or those who are immunocompromised. The presence of itching after urination could indicate that the infection is exacerbated by the urine's contact with the inflamed area.

3. Contact Dermatitis: This condition occurs when the skin reacts to irritants or allergens. If you have recently changed soaps, detergents, or personal care products, this could lead to an allergic reaction resulting in itching and discomfort.

4. Sexually Transmitted Infections (STIs): Certain STIs can cause similar symptoms. If you have had unprotected sexual contact, it is important to consider this possibility and seek testing.

5. Phimosis: If the foreskin is too tight and cannot be easily retracted, it can lead to irritation and inflammation, especially if hygiene is compromised. This condition can cause discomfort and itching.

Solutions and Recommendations:
- Hygiene: Maintain good hygiene by gently retracting the foreskin (if possible) and cleaning the area with mild soap and water. Avoid harsh soaps or products that may irritate the skin.

- Topical Treatments: Over-the-counter antifungal creams may help if a yeast infection is suspected. If you suspect contact dermatitis, hydrocortisone cream may reduce inflammation.

- Avoid Irritants: Identify and avoid any potential irritants, such as scented soaps, lotions, or laundry detergents.

- Consult a Healthcare Provider: Given the persistence of your symptoms, it is advisable to consult a healthcare provider. They may perform a physical examination, take a medical history, and possibly conduct tests to determine the exact cause of your symptoms. This may include swabs to check for infections or other underlying conditions.

- Consider STI Testing: If there is any chance of exposure to STIs, it is important to get tested to rule out any sexually transmitted infections.

- Follow-Up Care: If initial treatments do not alleviate your symptoms, follow up with your healthcare provider for further evaluation. They may refer you to a urologist for specialized care.

In summary, while post-urination itching and discomfort under the foreskin can stem from various causes, proper hygiene, avoidance of irritants, and consultation with a healthcare provider are essential steps toward finding relief and addressing the underlying issue.
