Ankylosing Spondylitis
Hello Doctor, I have been diagnosed with spondylitis.
Currently, I experience soreness and occasional sharp pain around my left and right scapula in the morning, and I also have some discomfort in my ribs.
Recently, I have been undergoing acupuncture and swimming.
I would like to ask what else I can do to alleviate or treat these symptoms.
Would you recommend massage therapy? I have read articles indicating that some people benefit from it while others do not.
I have been experiencing soreness in my right scapula upon waking, and occasionally both sides feel uncomfortable.
I am not currently taking any medication, and since my diagnosis of spondylitis, I have not had any flare-ups until these past few months when the discomfort began.

Hello, thank you for your question.
In the case of early-stage ankylosing spondylitis, there typically won't be any joint adhesions yet.
Therefore, in addition to exercise, manual therapy should be acceptable.
However, it is best to have your physician review your X-rays and discuss whether manual therapy is appropriate if there are no adhesions.
Additionally, if symptoms persist despite acupuncture and exercise, a comprehensive assessment of the overall disease status, including inflammatory markers, is necessary.
If needed, medication should be considered, in conjunction with exercise rehabilitation and acupuncture, which may yield better results.
After symptoms improve, you can discuss the possibility of reducing or discontinuing medication with your physician.
Best regards from the Allergy, Immunology, and Rheumatology Department of Changhua Hospital, Ministry of Health and Welfare.

Managing Ankylosing Spondylitis (AS) can be challenging, especially when symptoms like pain and stiffness in the shoulders and ribcage arise. It's great to hear that you are already engaging in acupuncture and swimming, as these can be beneficial for managing symptoms. Here’s a comprehensive overview of effective relief strategies and treatments for Ankylosing Spondylitis.
Understanding Ankylosing Spondylitis
Ankylosing Spondylitis is a type of inflammatory arthritis that primarily affects the spine and the sacroiliac joints in the pelvis. It can lead to severe, chronic pain and discomfort, particularly in the lower back and hips. Over time, it may cause the vertebrae to fuse, resulting in a rigid spine. Early diagnosis and treatment are crucial in managing symptoms and preventing complications.
Effective Relief Strategies
1. Physical Therapy: Engaging in a structured physical therapy program can significantly improve flexibility and strength. A physical therapist can design a personalized exercise regimen that focuses on stretching and strengthening the back and core muscles. This can help alleviate pain and improve posture.
2. Exercise: Regular exercise is essential for managing AS. Swimming, as you mentioned, is an excellent low-impact activity that can help maintain mobility without putting too much strain on the joints. Other beneficial exercises include walking, cycling, and yoga, which can enhance flexibility and reduce stiffness.
3. Medication: While you are currently not on medication, it may be worth discussing with your healthcare provider the possibility of starting nonsteroidal anti-inflammatory drugs (NSAIDs) to help manage pain and inflammation. In more severe cases, disease-modifying antirheumatic drugs (DMARDs) or biologics may be recommended.
4. Acupuncture: Continuing with acupuncture can be beneficial. Some studies suggest that acupuncture may help reduce pain and improve function in individuals with chronic pain conditions, including AS. It’s important to ensure that your acupuncturist is experienced in treating AS.
5. Heat and Cold Therapy: Applying heat can help relax tense muscles and alleviate pain, while cold therapy can reduce inflammation and numb sharp pain. You might find alternating between heat and cold to be effective.
6. Posture Awareness: Maintaining good posture is crucial in managing AS. Being mindful of your posture while sitting, standing, and sleeping can help reduce strain on your spine and alleviate discomfort.
7. Massage Therapy: While opinions on massage vary, many individuals with AS find relief through therapeutic massage. It can help reduce muscle tension and improve circulation. However, it’s essential to communicate your condition to the massage therapist so they can tailor the treatment accordingly.
8. Diet and Lifestyle: A balanced diet rich in anti-inflammatory foods (like fruits, vegetables, whole grains, and omega-3 fatty acids) can support overall health. Staying hydrated and avoiding smoking can also have positive effects on your condition.
9. Mind-Body Techniques: Practices such as mindfulness, meditation, and deep breathing exercises can help manage pain and improve your overall sense of well-being.
Considerations for Treatment
- Consultation with a Rheumatologist: Since AS is a systemic condition, regular check-ups with a rheumatologist are essential. They can monitor your condition and adjust your treatment plan as necessary.
- Avoiding Certain Activities: While some forms of physical activity are beneficial, high-impact sports or activities that involve heavy lifting may exacerbate symptoms and should be approached with caution.
- Monitoring Symptoms: Keep track of your symptoms and any changes you experience. This information can be valuable for your healthcare provider in adjusting your treatment plan.
Conclusion
Managing Ankylosing Spondylitis requires a multifaceted approach that includes physical activity, medication, and lifestyle modifications. While acupuncture and swimming are excellent choices, incorporating physical therapy, medication, and mindfulness practices can further enhance your quality of life. Always consult with your healthcare provider before making significant changes to your treatment plan, especially if you are considering new therapies like massage or medication. With the right strategies, you can effectively manage your symptoms and maintain an active lifestyle.
