Sharp, lightning-like needle sensations?

Hello Dr.
Jiang: In the past week, I have been experiencing intermittent sharp, lightning-like pain on the inner side of my left ankle.
The frequency is irregular, and if it occurs while I am sleeping, it wakes me up.
I also feel a crawling or tingling sensation in my limbs.
Today, I noticed a similar lightning-like prickling sensation on the inner side of my right ankle, although it is not as intense as the pain in my left ankle.
I consulted a neurologist, who suggested that it might be a bone fracture and recommended that I see an orthopedic specialist.
However, I do not feel any pain while walking briskly every day, and the discomfort mostly occurs when I am sitting.
What could this condition be, and which specialty should I consult? Thank you!

Dr. Jiang Junyi reply Neurology

Dear Ms.
Yen,
Based on your description of experiencing a crawling sensation in your limbs and a lightning-like prickling feeling on the inner side of your right ankle, it is important to consider the possibility of lumbar nerve root compression.
Symptoms of peripheral nerve compression typically include:
1.
Pain characterized by burning or electric sensations.
2.
Symptoms that have a specific dermatomal distribution, which can be triggered by touch or pressure.
3.
Accompanied by muscle soreness and weakness.
Examinations for peripheral nerve issues often utilize electrophysiological tests to assist in diagnosis.
Nerve conduction studies assess the conduction velocity and waveform of large fiber nerves, while electromyography evaluates the muscle response in the affected nerve distribution areas.

I recommend that you visit a neurology clinic for further evaluation, preferably with a physician specializing in peripheral nerve disorders, as they have more experience with electromyography testing.
Thank you for your inquiry.
If you have any further questions, feel free to discuss them!
Wishing you good health and peace,
Dr.

More Info

Dear Patient,
Thank you for reaching out with your concerns regarding the sharp, lightning-like pain in your ankle. Based on your description, it sounds like you are experiencing intermittent, acute pain localized to the inner side of your left ankle, with similar sensations beginning in your right ankle. Additionally, you mentioned experiencing a tingling sensation in your limbs, which can be quite distressing, especially if it disrupts your sleep.

There are several potential causes for the symptoms you are describing. Here are some possibilities:
1. Nerve Compression or Irritation: The sharp, lightning-like pain could be indicative of nerve compression or irritation. Conditions such as tarsal tunnel syndrome, where the tibial nerve is compressed as it passes through the tarsal tunnel near the ankle, can lead to similar symptoms. This condition often presents with pain, tingling, or a burning sensation in the foot and ankle.

2. Tendonitis or Tendon Injury: Inflammation of the tendons around the ankle, such as posterior tibial tendonitis, can cause sharp pain, especially during certain movements or after periods of inactivity. If you have been engaging in activities that put stress on your ankle, this could be a contributing factor.

3. Ligament Injury: Although you mentioned that you have not experienced pain during your daily activities, it is still possible to have a mild ligament injury or sprain that may not be apparent during movement but can cause pain when at rest.

4. Referred Pain: Sometimes, pain in the ankle can be referred from other areas, such as the lower back or hip. Conditions like sciatica can cause radiating pain down the leg, which may be perceived as originating from the ankle.

5. Circulatory Issues: The sensation of "ants crawling" on your skin could indicate a circulatory issue or peripheral neuropathy, which can be caused by various factors, including diabetes, vitamin deficiencies, or other systemic conditions.

Given that your symptoms have been persistent and are affecting your sleep, it is advisable to seek further evaluation. Since you have already consulted with a neurologist, I recommend following up with an orthopedic specialist, as they can assess for any structural issues in the ankle, such as fractures or tendon injuries. An MRI or ultrasound may be warranted to visualize the soft tissues and assess for any abnormalities.

In the meantime, here are some general recommendations:
- Rest: Avoid activities that exacerbate the pain, especially if you notice that sitting or resting triggers the symptoms.

- Ice Therapy: Applying ice to the affected area can help reduce inflammation and alleviate pain.

- Elevation: Keeping your foot elevated can help reduce swelling and improve circulation.

- Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help manage your discomfort.

If your symptoms worsen or if you experience additional symptoms such as swelling, bruising, or changes in mobility, it is crucial to seek immediate medical attention.
I hope this information helps you understand your condition better and guides you toward the appropriate next steps. Please take care, and I wish you a speedy recovery.

Sincerely,
Doctor Q&A Teams
