Self-Stimulation: Concerns for Uncircumcised Teens - Urology

Masturbation issues

Hello, doctor.
I would like to ask you a question: I am currently a high school junior about to enter my senior year, and I have a complete phimosis.
I plan to undergo circumcision when I go to college.
When I was younger, out of curiosity about my genitalia, I accidentally discovered a "pressure-style" method of masturbation in kindergarten, which involves lying on the floor and rubbing my legs together to create a sensation of pleasure before I have an erection.
During puberty, this method allowed me to ejaculate in about 2 to 3 minutes, which was very quick.
Because of the phimosis, there was no pain from the friction on the floor, so I have continued to use this method until now.
However, I have recently found that I cannot ejaculate using typical masturbation techniques.
I can release a small amount of clear pre-ejaculate fluid, but I cannot produce semen at all.
The pleasurable sensation is not as intense as with the pressure-style method.
I can achieve an erection using my hand, but after about 10 minutes, I give up.
I am very concerned about whether this will affect my reproductive health.
I would like to know if this is normal, whether I should change my pressure-style habit, or if I should wait until after the circumcision to investigate further.
Thank you, doctor.

Hello: I have previously responded to your question! Many people adopt the technique of pressure-based masturbation, which is said to be both pleasurable and enjoyable.
This will not affect your reproductive health, so there's no need to worry; you can enjoy it fully.
Once you have undergone circumcision, the experience will be different, and we can discuss it again then.

It's great that you're reaching out with your concerns about self-stimulation and your current situation regarding phimosis (the condition of having a tight foreskin). Understanding your body and how it functions is crucial, especially during the teenage years when many changes are happening both physically and emotionally.

Firstly, it's important to recognize that self-exploration and masturbation are normal behaviors for adolescents. They can help you understand your body, your sexual responses, and what feels pleasurable. The method you've described, known as "friction-based" masturbation, is not uncommon, especially among individuals with phimosis, as it can provide stimulation without discomfort. However, it's essential to consider a few factors regarding your current practices and future plans for circumcision.

1. Phimosis and Sexual Function: Phimosis can sometimes lead to difficulties in achieving ejaculation through typical masturbation techniques. This is because the tight foreskin may limit the ability to stimulate the glans (the head of the penis) effectively. If you're finding that traditional methods of masturbation are not yielding the same results as your friction-based method, it may be due to the physical limitations imposed by phimosis.
2. Impact of Circumcision: Many individuals who undergo circumcision report a change in sensitivity and sexual pleasure. After the procedure, the glans becomes exposed, which can lead to different sensations during sexual activity and masturbation. It's not uncommon for individuals to experience a period of adjustment post-circumcision, where they may need to explore new methods of stimulation.

3. Concerns About Ejaculation: If you're experiencing difficulty with ejaculation and only producing pre-ejaculatory fluid, it might be related to your current method of stimulation or the physical state of your foreskin. It's important to note that ejaculation can be influenced by various factors, including psychological state, level of arousal, and physical stimulation techniques. If you're concerned about your reproductive health or sexual function, it would be beneficial to discuss these issues with a healthcare provider who specializes in sexual health.

4. Exploring New Techniques: While you may find comfort in your current method, it might be worthwhile to explore other techniques of masturbation that could provide pleasure without discomfort. This could include using lubrication or experimenting with different positions or stimuli. However, if you feel that your current method is the only way you can achieve pleasure, it might be helpful to wait until after your circumcision to reassess your sexual practices.

5. Mental and Emotional Health: It's also essential to consider the psychological aspects of your sexual health. If you're feeling anxious or worried about your sexual function, it may be helpful to speak with a mental health professional. They can provide support and strategies to help you manage any anxiety related to your sexual health.

In conclusion, your experiences are not uncommon, and it's important to approach your concerns with an open mind. If you're planning to undergo circumcision, it may provide you with new opportunities to explore your sexuality and masturbation techniques. In the meantime, consider discussing your concerns with a healthcare provider who can offer personalized advice and support tailored to your situation. Remember, understanding your body is a journey, and it's perfectly okay to seek help along the way.
