Concerns about premature ejaculation?

This issue has been bothering me for a long time.
If I refrain from masturbation for about a week, I experience nocturnal emissions because I tend to turn over in my sleep, which may compress or press on my penis.
I can even feel the nocturnal emission while dreaming.
There was even a time when I experienced it while sleeping on my stomach.
My masturbation sessions are usually quite short, although occasionally I manage to control the duration a bit longer.
When the sessions are shorter, the sensation of ejaculation feels like it is coming from the very tip of the penis, and I can't seem to stop it.
When I manage to last longer, the sensation during ejaculation feels like it is coming from deeper inside, and I feel more in control.
I have also tried the stop-start technique recommended by some doctors online, but when I feel the urge to ejaculate and pause for a few seconds before continuing, I find that I quickly feel the urge to ejaculate again.
Am I experiencing premature ejaculation? Should I see a doctor for an examination? Thank you for your help.

Dr. Fu Shizhang reply Urology

According to the definition by the International Society for Sexual Medicine (ISSM), premature ejaculation is generally defined as ejaculation occurring within one minute of vaginal penetration.
Masturbation duration is generally not considered within the scope of premature ejaculation.
Online consultations cannot replace an in-person medical interview and physical examination by a physician; it is still recommended to seek medical attention at healthcare facilities when there are health concerns.

More Info

Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation during sexual activity, leading to ejaculation that occurs sooner than desired, often with minimal sexual stimulation. Understanding the symptoms, causes, and potential solutions can help you address this concern effectively.

Symptoms of Premature Ejaculation
The primary symptom of PE is the inability to delay ejaculation, which can occur before, during, or shortly after penetration. This can lead to feelings of frustration, embarrassment, and anxiety, which may further exacerbate the issue. Other symptoms may include:
1. Inconsistent Control: Some men may experience varying levels of control over ejaculation, where they can sometimes delay it but struggle at other times.

2. Ejaculation with Minimal Stimulation: As you mentioned, if you find that ejaculation occurs with little stimulation or after a very short duration of sexual activity, this may indicate PE.

3. Emotional Distress: Feelings of inadequacy, anxiety about sexual performance, or frustration with sexual encounters can accompany PE.

Causes of Premature Ejaculation
The causes of PE can be multifactorial, including:
1. Psychological Factors: Anxiety, stress, and performance pressure can significantly impact sexual function. Past experiences, such as early sexual encounters or negative feelings about sex, can also contribute.

2. Biological Factors: Hormonal imbalances, neurotransmitter levels, or sensitivity of the penis may play a role in PE. Conditions such as prostatitis or thyroid issues can also affect ejaculation.

3. Relationship Issues: Communication problems or unresolved conflicts with a partner can lead to anxiety during sexual encounters, contributing to PE.

Solutions and Treatment Options
If you suspect that you are experiencing PE, it is advisable to consult a healthcare professional for a thorough evaluation. Here are some potential solutions and treatment options:
1. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you gain better control over ejaculation. These methods involve pausing sexual activity or applying pressure to the penis to delay ejaculation.

2. Desensitizing Products: Over-the-counter creams or sprays that contain numbing agents can reduce sensitivity and help prolong the time before ejaculation.

3. Counseling or Therapy: Speaking with a therapist or counselor can help address any underlying psychological issues, such as anxiety or relationship problems. Cognitive-behavioral therapy (CBT) has been shown to be effective for many men experiencing PE.

4. Medications: Certain medications, including selective serotonin reuptake inhibitors (SSRIs), have been found to delay ejaculation as a side effect. Your doctor may prescribe these if appropriate.

5. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises (Kegel exercises) can improve control over ejaculation.

When to See a Doctor
If your symptoms persist or worsen, or if they significantly impact your quality of life or relationships, it is essential to seek medical advice. A healthcare provider can help rule out any underlying medical conditions and work with you to develop a tailored treatment plan.

In conclusion, premature ejaculation is a manageable condition, and many men experience it at some point. By understanding the symptoms and causes, and exploring various treatment options, you can take steps toward improving your sexual health and overall well-being. Don't hesitate to reach out to a healthcare professional for support and guidance tailored to your specific situation.
