Post-Surgery Bleeding and Discomfort: Is It Normal After Hemorrhoid Surgery? - Surgery

Occasional bleeding after hemorrhoid surgery?

Thank you for your previous response.
The issue of redness, swelling, and itching has improved after changing medication.
It has been almost two months since my surgery on June 15.
Occasionally, I notice a little blood after bowel movements, along with some stinging pain.
There is also some clear discharge, which is minimal and odorless.
I observed that a specific area around the anal opening is quite red, differing somewhat from the surrounding pink tissue.
The surgeon who performed the operation indicated that there is no problem and advised that I do not need to return for a follow-up.
Dr.
Zhang, is this normal after almost two months? What can I do to improve this situation, or should I seek another physician for an examination?

He Xiaowen, 30~39 year old female. Ask Date: 2023/08/07

Dr. Zhang Shuhao reply Surgery

Typically, new anal fissures can occur due to friction at the surgical wound site or from straining during bowel movements.
It is advisable to take warm sitz baths, increase fluid intake, and consume dietary fiber.
If there is no improvement, further evaluation should be sought.

Post-surgery bleeding and discomfort can be common experiences following hemorrhoid surgery, but it's essential to differentiate between what is considered normal and what may indicate a complication. After hemorrhoid surgery, particularly procedures like the PPH (Procedure for Prolapse and Hemorrhoids), patients may experience some bleeding, discomfort, and changes in bowel habits as part of the healing process.

In your case, experiencing slight bleeding after bowel movements and some discomfort, such as a stinging sensation, can be within the realm of normal post-operative symptoms. However, the presence of a significant amount of blood, especially if it is bright red, should always be taken seriously. The fact that you have noticed a specific area that appears redder than the surrounding tissue could suggest localized irritation or inflammation, which is not uncommon after surgery.

The transparent discharge you mentioned is also not unusual. This could be a sign of healing tissue or a mild inflammatory response. However, if the discharge changes in color, becomes foul-smelling, or increases in volume, it could indicate an infection that would require medical attention.

Given that it has been nearly two months since your surgery, it is advisable to consult with a healthcare professional if you have ongoing concerns. While your original surgeon may have indicated that everything is healing well, it is always within your rights as a patient to seek a second opinion, especially if you feel that your symptoms are not improving or if you have new concerns.

In terms of management, here are some general recommendations that may help improve your condition:
1. Dietary Adjustments: Ensure you are consuming a high-fiber diet to promote soft stools and reduce straining during bowel movements. This can help minimize discomfort and bleeding.

2. Hydration: Drink plenty of fluids to help keep your stools soft.

3. Sitz Baths: Regular sitz baths can help soothe the anal area and promote healing. Warm water can help reduce discomfort and improve blood circulation to the area.

4. Topical Treatments: Over-the-counter creams or ointments designed for hemorrhoid relief may provide symptomatic relief. However, consult with your healthcare provider before starting any new treatments.

5. Avoid Straining: Try to avoid straining during bowel movements, as this can exacerbate bleeding and discomfort.

6. Follow-Up Care: If symptoms persist or worsen, or if you notice any new symptoms such as fever, increased pain, or significant changes in your bowel habits, it is crucial to seek medical attention promptly.

In conclusion, while some bleeding and discomfort can be normal after hemorrhoid surgery, persistent or worsening symptoms warrant further evaluation. Consulting with a healthcare professional can provide you with peace of mind and ensure that any potential complications are addressed promptly. Remember, your health and comfort are paramount, and seeking a second opinion is always a valid option if you feel uncertain about your recovery.
