Masturbation-induced premature ejaculation?

Doctor, I have had a habit of masturbating almost every day from seventh grade to high school.
Suddenly, one day I ejaculated in less than three minutes.
After not masturbating for a day, I did it twice the next day, and both times I ejaculated quickly.
Is this premature ejaculation? I didn't have this problem before.
After summer vacation, I started going to bed late, drinking a lot of sugary drinks, and very little water.
I exercised in July and early August, but then I stopped.
Could this be affecting me? How can I improve this? Will it get better on its own, or do I need to see a doctor?

Dr. Cai Zhuorong reply Urology

Premature ejaculation is determined based on sexual intercourse, not on masturbation.

More Info

Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by ejaculation that occurs with minimal sexual stimulation, often before or shortly after penetration, leading to distress or frustration for both partners. Understanding the causes, effects, and potential solutions for PE can help individuals manage this condition effectively.

Causes of Premature Ejaculation
1. Psychological Factors: Anxiety, stress, and depression are significant contributors to PE. Performance anxiety, in particular, can create a cycle where the fear of premature ejaculation leads to increased anxiety, which in turn exacerbates the problem.

2. Physiological Factors: Hormonal imbalances, neurotransmitter levels, and certain medical conditions (like prostatitis or thyroid issues) can influence ejaculation timing. Additionally, sensitivity of the glans penis can play a role; some men may have heightened sensitivity that leads to quicker ejaculation.

3. Lifestyle Factors: Changes in lifestyle, such as increased consumption of alcohol or recreational drugs, can affect sexual performance. In your case, the shift in your routine, including late nights and changes in hydration, might have contributed to your experience.

4. Relationship Issues: Problems within a relationship, such as lack of communication or unresolved conflicts, can also impact sexual performance and satisfaction.

Effects of Premature Ejaculation
The effects of PE can be both physical and emotional. Physically, it may lead to dissatisfaction in sexual encounters, which can strain relationships. Emotionally, it can cause feelings of inadequacy, embarrassment, and frustration, potentially leading to avoidance of sexual intimacy altogether.

Solutions and Management
1. Self-Assessment: It’s essential to assess whether this is a persistent issue or a temporary phase. Since you mentioned that this has been a recent occurrence after a period of daily masturbation and changes in your lifestyle, it may be a temporary issue related to anxiety or physical changes.

2. Behavioral Techniques: Techniques such as the "stop-start" method or the "squeeze" technique can help you gain better control over ejaculation. These methods involve pausing during sexual activity or applying pressure to the penis to delay ejaculation.

3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through Kegel exercises can improve control over ejaculation. These exercises involve repeatedly contracting and relaxing the muscles that control urination.

4. Lifestyle Modifications: Improving your overall health by maintaining a balanced diet, staying hydrated, and engaging in regular physical activity can positively impact sexual health. Reducing alcohol and caffeine intake may also help.

5. Professional Help: If the problem persists or worsens, consulting a healthcare professional is advisable. A doctor can provide a thorough evaluation and may recommend therapy, medications, or other interventions tailored to your specific situation.

6. Communication: Open communication with your partner about your experiences and feelings can help alleviate anxiety and improve intimacy.

Conclusion
In summary, premature ejaculation can be influenced by a variety of factors, including psychological, physiological, and lifestyle changes. Given your recent experiences, it may be beneficial to monitor the situation for a while, implement some of the self-help strategies mentioned, and consider seeking professional advice if the issue continues. Remember, you are not alone in this, and many men experience similar challenges. Addressing the issue with understanding and care can lead to improved sexual health and relationship satisfaction.
