Phimosis
Hello Doctor: I am 28 years old and currently have a steady partner.
I would like to ask about something I discovered about three months ago.
I found two small growths at the urethral opening of my foreskin, about the size of half a grain of rice.
They feel slightly tender when touched.
What could this be? Additionally, I feel that I have phimosis and would like to undergo a circumcision.
How long does the surgery typically take, and what type of procedure is it? Is the cost covered by insurance or is it out-of-pocket? If it is out-of-pocket, what is the approximate amount? Will the surgery affect my ability to work afterward? I appreciate your assistance in answering these questions.
Thank you.

Hello Jonathan: The rash you described may likely be pearly penile papules.
The circumcision procedure takes about thirty minutes and is performed under local anesthesia.
The out-of-pocket cost is approximately 1600 to 1700 NTD.
You may experience some discomfort for 3 to 5 days post-surgery.

More Info
Phimosis is a condition where the foreskin of the penis cannot be easily retracted over the glans (the head of the penis). It can be a source of discomfort and may lead to complications if not addressed. In your case, the presence of small growths at the urethral opening, along with your concerns about phimosis, warrants a thorough evaluation by a healthcare professional.
Symptoms of Phimosis
Common symptoms associated with phimosis include:
- Difficulty retracting the foreskin, especially during erections.
- Pain or discomfort during sexual activity.
- Inflammation or infection of the glans (balanitis).
- The presence of growths or lesions, as you described, which could indicate other conditions such as genital warts or pearly penile papules.
Surgical Options
If phimosis is causing significant discomfort or recurrent infections, circumcision (the surgical removal of the foreskin) is often recommended. The procedure typically lasts about 30 to 60 minutes, depending on the complexity and the surgeon's technique.
There are also less invasive options, such as preputioplasty, which involves widening the opening of the foreskin without complete removal. This can be a suitable alternative for those who wish to preserve the foreskin.
Costs and Insurance Coverage
In terms of costs, circumcision can vary widely based on geographic location, the surgeon's experience, and whether the procedure is performed in a hospital or an outpatient setting. In many cases, circumcision is covered by health insurance if it is deemed medically necessary, but this can vary by plan. If it is considered elective, you may need to pay out-of-pocket. The costs can range from a few hundred to several thousand dollars, depending on the factors mentioned.
Recovery and Impact on Daily Life
Post-operative recovery from circumcision typically involves some discomfort, swelling, and bruising, which can last for a few days to a week. Most patients can return to normal activities, including work, within a week, but it is advisable to avoid strenuous activities and sexual intercourse for at least four to six weeks to allow for proper healing.
Recommendations
Given your symptoms and concerns, it is crucial to consult a urologist or a healthcare provider specializing in male reproductive health. They can perform a physical examination, possibly including a biopsy of the growths if necessary, to determine their nature and whether they are related to phimosis or another condition.
In summary, phimosis can lead to various complications, and surgical intervention may be warranted based on your symptoms. Understanding the procedure, costs, and recovery process will help you make an informed decision. Always discuss your concerns and treatment options with a qualified healthcare professional to ensure the best outcome for your health.
