Anxiety disorder, fatigue, panic?
Hello Doctor, I have experienced sudden fatigue and a feeling of weakness at work several times.
Could this be related to anxiety disorder? I often feel tense, and sometimes I experience a sense of weakness afterward.
Additionally, I frequently wake up feeling tired and lacking energy, with overall body weakness.
There have been instances where I suddenly had panic attacks while out, and now I am hesitant to go outside because I feel very anxious.
Are there any methods or practices I can try to improve this? Furthermore, I have previously taken Metoprolol and Escitalopram.
I took Metoprolol for six months but felt it was not effective, and I stopped Escitalopram due to severe nausea.
Could you recommend any other medications I might try?

Hello, thank you for reaching out and sharing your experiences. It sounds like you are dealing with a significant amount of anxiety, which can manifest in various ways, including feelings of fatigue, panic attacks, and a general sense of unease. Your symptoms, such as sudden feelings of exhaustion, panic attacks, and persistent fatigue even after sleep, can indeed be related to anxiety disorders.
Anxiety can lead to physical symptoms that mimic exhaustion or fatigue. When you're anxious, your body is in a constant state of heightened alertness, which can drain your energy and lead to feelings of fatigue. This is often referred to as "adrenal fatigue," where the body's stress response is overactive, leading to a cycle of anxiety and exhaustion. Additionally, panic attacks can cause a rush of adrenaline, which may leave you feeling drained afterward.
It's also important to note that sleep disturbances are common in individuals with anxiety. If you are waking up feeling unrefreshed, it may be due to the quality of your sleep being affected by anxiety. Anxiety can lead to restless sleep, frequent awakenings, or even insomnia, which can exacerbate feelings of fatigue during the day.
To manage your anxiety and improve your overall well-being, consider the following strategies:
1. Cognitive Behavioral Therapy (CBT): This form of therapy is highly effective for anxiety disorders. It helps you identify and challenge negative thought patterns and develop healthier coping mechanisms. A therapist can guide you through this process.
2. Mindfulness and Relaxation Techniques: Practices such as mindfulness meditation, deep breathing exercises, and progressive muscle relaxation can help reduce anxiety and improve your ability to cope with stress. These techniques can also enhance your sleep quality.
3. Regular Physical Activity: Engaging in regular exercise can significantly reduce anxiety levels. Exercise releases endorphins, which are natural mood lifters. Aim for at least 30 minutes of moderate exercise most days of the week.
4. Healthy Sleep Hygiene: Establish a calming bedtime routine, avoid screens before bed, and create a comfortable sleep environment. This can help improve the quality of your sleep.
5. Nutrition: A balanced diet can impact your mood and energy levels. Ensure you are eating a variety of foods rich in vitamins and minerals, and stay hydrated.
6. Medication: Since you have previously tried medications like Metoprolol (美妥平) and Clonazepam (克憂果) with limited success, it may be beneficial to discuss alternative medications with your healthcare provider. There are various classes of medications for anxiety, including SSRIs (Selective Serotonin Reuptake Inhibitors) and SNRIs (Serotonin-Norepinephrine Reuptake Inhibitors), which might be more suitable for you. Your doctor can help find a medication that works for you with manageable side effects.
7. Support System: Connecting with friends, family, or support groups can provide emotional support and help you feel less isolated. Sharing your experiences with others who understand can be very therapeutic.
8. Gradual Exposure: If you are experiencing panic attacks and fear of going out, consider gradually exposing yourself to situations that trigger your anxiety. Start with small steps and gradually increase your exposure as you become more comfortable.
It's crucial to consult with a mental health professional who can provide a comprehensive evaluation and tailor a treatment plan that suits your specific needs. Remember, managing anxiety is a process, and it's okay to seek help along the way. You are not alone in this, and with the right support and strategies, you can find relief from your symptoms and improve your quality of life.
