Glans penis swelling and pain?


Recently, I have been experiencing a feeling of extreme engorgement in the glans during erections, to the point of discomfort.
By the time I reach ejaculation, the glans feels so swollen that it is very uncomfortable, and I have also noticed that the size of the urethral opening appears larger than before during erections.
I am unsure what could be causing this.
