Flashing lights in the eyes upon waking?
Hello, I would like to ask about a condition I've been experiencing.
Since May, every time I wake up, I have a flickering sensation in my eyes that lasts for about ten seconds, which only goes away after I blink.
Additionally, I started noticing floaters in my vision.
In August, I underwent various examinations at Taichung Veterans General Hospital's ophthalmology department, all of which came back normal.
I was then referred to the neurology department at the same hospital, where all tests, including an MRI of the head, were also normal.
However, the symptoms persist; every time I wake up, I still experience this flickering sensation for about ten seconds, which disappears after blinking.
This has been troubling me for five months.
Thank you, doctor, for helping to clarify my concerns.

Hello: The sensations of flickering and floaters you described may be common issues related to the eyes and visual system.
However, since you have undergone multiple examinations by both ophthalmology and neurology without any abnormalities found, these symptoms may not indicate a significant problem and can sometimes be physiological phenomena.
1.
Photophobia: The flickering sensation may be due to light sensitivity, especially when transitioning suddenly from a dark environment to a bright one or upon waking.
This is a normal physiological response in some cases, but if the discomfort is pronounced, you might try to avoid sudden bright lights and use appropriate indoor lighting.
2.
Floaters: Floaters are small particles or fibers within the vitreous of the eye that drift in your field of vision, often more noticeable against a bright background.
This is quite common among many individuals and usually does not require treatment.
If both your ophthalmologist and neurologist have found no issues, this may be a harmless phenomenon.
3.
Retinal Examination: Consider regular retinal examinations to ensure the health of your retina.
Occasionally, the sensation of floaters may be an early sign of retinal issues, but typically requires further evaluation by a retinal specialist.
4.
Healthy Lifestyle: Maintaining good lifestyle habits, including adequate sleep, a balanced diet, and moderate exercise, can help support eye health and overall well-being.
5.
Visual Habits: Try to reduce prolonged screen time, take regular breaks for your eyes, and ensure your screen settings are at appropriate brightness and contrast levels.

Experiencing eye flickering upon waking can be quite disconcerting, especially when it persists for several months. Based on your description, it seems that you have undergone various examinations, including eye tests and MRI scans, which have not revealed any significant underlying issues. This is a positive sign, as it suggests that more serious conditions have been ruled out. However, the persistence of your symptoms warrants further exploration of potential causes and solutions.
Possible Causes of Eye Flickering
1. Fatigue and Sleep Quality: One of the most common reasons for eye flickering or visual disturbances upon waking is fatigue. If you are not getting enough restful sleep, your eyes may not have the opportunity to recover fully. Poor sleep quality can lead to eye strain, which may manifest as flickering or other visual disturbances.
2. Dry Eyes: Upon waking, your eyes may be dry, especially if you sleep in a room with low humidity or if you are a mouth breather. Dryness can lead to temporary visual disturbances, including flickering. The act of blinking helps to distribute tears and can alleviate this sensation.
3. Visual Fatigue: If you spend a lot of time on screens or reading before bed, your eyes may be fatigued. This fatigue can lead to temporary issues with focus and clarity upon waking. The flickering sensation may be your eyes adjusting to light and focus after a period of rest.
4. Neurological Factors: Although your neurological examinations have returned normal results, it is worth noting that sometimes, transient visual disturbances can be linked to neurological factors, such as migraines or other conditions that may not always show up on standard tests.
5. Eye Conditions: Conditions such as vitreous floaters (the "flying mosquitoes" you mentioned) can also cause visual disturbances. While these are usually harmless, they can be bothersome and may contribute to the sensation of flickering.
Solutions and Recommendations
1. Improve Sleep Hygiene: Focus on improving your sleep environment. Ensure that your bedroom is dark, quiet, and cool. Establish a regular sleep schedule and avoid screens at least an hour before bedtime to help your eyes and brain prepare for sleep.
2. Stay Hydrated: Ensure you are drinking enough water throughout the day. Dehydration can contribute to dry eyes, which may exacerbate flickering sensations.
3. Use Artificial Tears: If you suspect dry eyes, consider using lubricating eye drops (artificial tears) before bed and upon waking. This can help keep your eyes moist and reduce flickering.
4. Limit Screen Time: If you spend long hours on screens, take regular breaks using the 20-20-20 rule: every 20 minutes, look at something 20 feet away for 20 seconds. This can help reduce eye strain.
5. Consult an Eye Specialist: If the flickering persists despite these measures, it may be beneficial to consult an ophthalmologist or optometrist for a comprehensive eye examination. They may consider additional tests or treatments tailored to your specific symptoms.
6. Follow Up with Neurology: Since you have already seen a neurologist, if your symptoms worsen or new symptoms arise, it may be worth a follow-up appointment to explore other potential neurological causes.
In conclusion, while the flickering sensation upon waking can be bothersome, it is often linked to fatigue, dry eyes, or visual strain. By implementing some lifestyle changes and consulting with healthcare professionals, you can work towards alleviating these symptoms. Remember, it is essential to listen to your body and seek help if your symptoms persist or worsen.
