Missed menstruation?
Hello Doctor, my girlfriend's previous menstrual periods were from July 20 to July 25 and from August 29 to September 4.
During September, we had multiple sexual encounters, all with condom use.
The app predicts her next period to be between September 25 and September 30.
However, on September 23, after the condom slipped, she took emergency contraception (Levonorgestrel) within 12 hours.
She experienced some side effects from the medication but did not have any bleeding, and she still has not gotten her period.
After sexual intercourse on October 6, she noticed some bleeding but without any pain.
Could this indicate that her period is approaching? Is there still a chance of pregnancy? Thank you, Doctor.

Hello, it is possible that your menstrual period is about to start.
If it does not arrive after another week, it is advisable to see a gynecologist.
Wishing you good health.

More Info
When it comes to missed periods and the possibility of pregnancy, it is essential to consider various factors, including the timing of sexual activity, contraceptive methods used, and the effects of emergency contraception. Based on the information provided, your girlfriend's menstrual cycle appears to have some irregularities, which can complicate the situation.
Firstly, the menstrual cycle can vary significantly from person to person and even from month to month for the same individual. The average menstrual cycle lasts about 28 days, but cycles can range from 21 to 35 days. In your girlfriend's case, her last known periods were from July 20 to July 25 and from August 29 to September 4. If we consider the average cycle length, her next expected period would be around September 29 to October 4. However, since she took emergency contraception (Plan B) on September 23, this could have affected her menstrual cycle.
Emergency contraception pills like "Levonorgestrel" (often referred to as "Plan B" or "Ella") can delay ovulation and, consequently, the onset of menstruation. It is not uncommon for women to experience changes in their menstrual cycle after taking these pills, including delayed periods, irregular bleeding, or spotting. The absence of menstruation following the use of emergency contraception does not necessarily indicate pregnancy, as hormonal fluctuations can lead to missed or delayed periods.
On October 6, you mentioned that there was some bleeding after sexual intercourse, which could be a sign of several things. This bleeding could be implantation bleeding if pregnancy occurred, but it could also be breakthrough bleeding caused by hormonal changes due to the emergency contraception. The absence of pain during this bleeding is also a good sign, as painful bleeding could indicate other issues, such as an ectopic pregnancy or a miscarriage.
Regarding the possibility of pregnancy, it is essential to consider that while condoms are effective in preventing pregnancy, they are not 100% foolproof. If the condom slipped or broke during intercourse, there is a chance of sperm exposure. However, since your girlfriend took emergency contraception within 12 hours of the incident, this significantly reduces the likelihood of pregnancy.
To clarify the situation, I recommend the following steps:
1. Wait for the Next Period: If your girlfriend's period does not arrive within a week or so of the expected date, she should take a home pregnancy test. These tests are most accurate when taken after a missed period.
2. Consult a Healthcare Provider: If there are any concerns about the bleeding or if she experiences any unusual symptoms (such as severe pain, heavy bleeding, or signs of infection), it is crucial to seek medical attention.
3. Monitor for Side Effects: After taking emergency contraception, it is essential to monitor for any side effects, including changes in menstrual patterns, nausea, or fatigue.
4. Consider Regular Contraceptive Methods: If your girlfriend is sexually active and wants to prevent pregnancy in the future, discussing regular contraceptive options with a healthcare provider would be beneficial.
In summary, while there is a possibility of pregnancy, the use of emergency contraception and the irregularities in her menstrual cycle complicate the situation. Monitoring her menstrual cycle and taking a pregnancy test if her period does not arrive will provide clarity. If there are any concerns or unusual symptoms, seeking medical advice is always the best course of action.
