Regarding COVID-19 vaccines?

Hello, Doctor! I would like to ask: I received my first dose of the COVID-19 vaccine (AstraZeneca) on September 10, 2021, my second dose (BioNTech) on November 24, 2021, and my booster dose (BioNTech) on March 1, 2022.
I would like to know what the outcomes would be if I do not receive any additional vaccinations and continue to live in Taiwan.
For example: 1.
Is there a risk of developing severe illness if I contract COVID-19? (I am 29 and a half years old, have no chronic diseases, only suffer from nasal allergies and a mental health condition.) 2.
What is the likelihood of experiencing long COVID symptoms? (For instance, brain fog or other cognitive function impairments.) Thank you, Doctor!

Niming, 20~29 year old female. Ask Date: 2023/10/29

Dr. Ye Qianyu reply Family Medicine

Hello! The circulating strains of the COVID-19 virus are rapidly mutating.
If you are concerned about contracting the virus and its complications, it is recommended that you receive the latest vaccine as advised by the Centers for Disease Control.
Currently, there is no method to predict the likelihood of severe illness.

Thank you for your questions regarding the impact of COVID-19 vaccination on health and the potential long-term effects of the virus. It's important to understand how vaccination and prior infections can influence your health outcomes, especially in the context of COVID-19.

1. Risk of Severe Disease: Given your vaccination history, you have received three doses of COVID-19 vaccines, which significantly enhances your immunity against severe disease. The vaccines, particularly mRNA vaccines like BNT (Pfizer-BioNTech), have been shown to be effective in reducing the risk of severe illness, hospitalization, and death from COVID-19. While no vaccine offers 100% protection, your age (29 years) and absence of chronic diseases further reduce your risk of developing severe symptoms if you were to contract the virus. However, it is still possible to experience mild to moderate symptoms, especially with the emergence of new variants that may partially evade vaccine-induced immunity.

2. Likelihood of Long COVID: The risk of developing long COVID, or post-acute sequelae of SARS-CoV-2 infection (PASC), can vary based on several factors, including the severity of the initial infection, underlying health conditions, and vaccination status. Studies indicate that vaccinated individuals who contract COVID-19 tend to have a lower risk of developing long-term symptoms compared to unvaccinated individuals. While your vaccination history is a protective factor, it does not completely eliminate the risk of long COVID. Symptoms such as "brain fog," fatigue, and other cognitive impairments have been reported in some individuals post-infection, regardless of vaccination status. However, the overall incidence of these symptoms appears to be lower in vaccinated populations.

3. Current Recommendations: Given that you live in Taiwan, where vaccination rates are relatively high and public health measures are in place, it is advisable to stay informed about any new variants and local health guidelines. If you are considering additional booster doses, consult with your healthcare provider about the latest recommendations, as booster shots can further enhance your immunity, especially against emerging variants.

4. Mental Health Considerations: You mentioned having a history of mental health conditions. It's essential to monitor your mental well-being, especially during and after any COVID-19 infection. Some individuals report exacerbation of mental health symptoms following infection, which may be related to the stress of illness, changes in routine, or the direct effects of the virus on the brain. Engaging in supportive therapies and maintaining a healthy lifestyle can be beneficial.

In summary, while your vaccination history provides a strong defense against severe COVID-19 and may reduce the risk of long COVID, it is still crucial to remain vigilant. Monitor your health, stay updated on vaccination recommendations, and seek medical advice if you experience any concerning symptoms. Your proactive approach to health is commendable, and staying informed will help you navigate any potential risks associated with COVID-19.
