Unexplained Nipple Holes: What Could Be Causing This Pain? - Family Medicine

The nipple has developed two unexplained large holes?

Hello, a couple of days ago I went to a hot spring and used an oven (I had no issues while bathing).
However, yesterday while showering, I noticed some pain in my nipples.
Upon inspection, I found two large holes (considering the size relative to the nipple).
Touching the area around the holes is painful, similar to the sensation of a scraped layer of skin.
The outer edge of the holes feels slightly raised and hard.
Today, while showering, the pain persisted, and I discovered two additional smaller holes: one is a raised black dot in the center of a ring without an open wound, and the other is a small pink hole that looks like a scraped layer of skin.
All the wounds are painful to the touch.
The sudden appearance of these holes is concerning, and I'm worried it might be an infection or some other condition.
Which type of doctor should I see: family medicine, dermatology, surgery, breast specialty, or obstetrics and gynecology? Or is there another specialty you would recommend? I couldn't find information online about my symptoms, and I'm feeling a bit anxious.
Thank you, doctor.

It sounds like you're experiencing a concerning issue with your nipples, specifically the sudden appearance of holes and associated pain. Given the symptoms you've described, it's important to approach this situation with caution and seek medical advice promptly.

The presence of unexplained nipple holes, especially when accompanied by pain, can be indicative of several potential conditions. These may include:
1. Infection: Conditions such as folliculitis or an abscess can cause localized pain and changes in the skin. If there is any redness, swelling, or discharge, this could suggest an infection that requires treatment.

2. Dermatitis: Skin conditions like eczema or contact dermatitis can lead to irritation and changes in the skin's appearance. If you've recently changed soaps, lotions, or other products, this could be a contributing factor.

3. Trauma: If you've engaged in activities that could have caused friction or injury to the nipple area (such as certain types of clothing or vigorous washing), this might explain the pain and skin changes.

4. Paget's Disease of the Nipple: Although less common, this condition can present with changes in the nipple and surrounding area, often associated with underlying breast cancer. It typically manifests with symptoms like itching, burning, or a discharge.

5. Other Dermatological Issues: Conditions such as psoriasis or other inflammatory skin diseases can also affect the nipple area.

Given the variety of potential causes, it is advisable to consult a healthcare professional. The best initial step would be to see a dermatologist or a breast specialist (often found in breast clinics or women's health centers), as they can provide a thorough examination and determine if a biopsy or further testing is necessary. If there are signs of infection, your primary care physician or a family doctor could also be a good starting point, as they can prescribe antibiotics if needed.

In the meantime, here are some general recommendations to help manage your symptoms:
- Avoid Irritants: Refrain from using harsh soaps or lotions on the affected area. Opt for gentle, fragrance-free products.

- Keep the Area Clean and Dry: Maintain good hygiene to prevent any potential infections from worsening.

- Avoid Scratching or Picking: This can exacerbate the condition and lead to further irritation or infection.

- Monitor Symptoms: Keep track of any changes in the appearance of the holes, pain levels, or any additional symptoms like fever or discharge.

It's understandable to feel anxious about these changes in your body, but seeking medical advice is the best course of action. Early intervention can lead to better outcomes and peace of mind. Remember, your health is paramount, and addressing these symptoms with a professional will provide you with the clarity and treatment you need.
