Postoperative care after laparoscopic myomectomy?

I underwent laparoscopic myomectomy on September 27, 2023, for a 13.1 cm uterine fibroid.
The day after the surgery, I received a long-acting injection of leuprolide.
I have a follow-up appointment one month post-surgery and have two questions for the doctor:
1.
During the follow-up, a transvaginal ultrasound was performed, and the doctor mentioned there is a "uterine cavity." I don't quite understand what this means.
The doctor only said that because my fibroid was relatively large, there would be this uterine cavity after suturing, and it would likely grow back in about six months.
Is this normal?
2.
Since the surgery, as of November 10, I have experienced a small amount of brown discharge, and there was also a small amount of brown discharge on November 15.
Is this normal?

More Info

After undergoing a laparoscopic myomectomy on September 27, 2023, you have raised some important questions regarding your recovery and the changes you are experiencing. Let’s address your concerns one by one, while also providing some context about the procedure and what to expect during the recovery phase.

1. Understanding the "Uterine Cavity": When your doctor mentioned the presence of a "uterine cavity" during your follow-up ultrasound, they were referring to the space within the uterus that is lined by the endometrium. After the removal of a large fibroid (in your case, 13.1 cm), the uterus may have a temporary cavity or space where the fibroid was located. This is normal, especially with larger fibroids, as the uterus needs time to heal and reshape itself. The fact that your doctor indicated it might take about six months for the uterus to return to a more typical shape is also common. During this healing period, the uterine tissue will gradually remodel, and the cavity may fill in as the endometrial lining regenerates. It’s essential to follow up with your healthcare provider to monitor the healing process.

2. Brown Discharge: The presence of slight brown discharge post-surgery can be a normal part of the healing process. This discharge often consists of old blood that is being expelled from the uterus as it heals. It is not uncommon to experience some spotting or discharge for several weeks after a myomectomy, especially if the fibroid was large. However, if the discharge becomes heavy, is accompanied by a foul odor, or is associated with significant pain, you should contact your healthcare provider immediately, as these could be signs of infection or other complications.

Additional Considerations for Recovery
- Pain Management: After a laparoscopic myomectomy, some degree of discomfort is expected as your body heals. It’s important to manage pain effectively, using prescribed medications as directed. If you experience sharp or worsening pain, it’s advisable to consult your doctor.

- Activity Level: While it’s crucial to rest, gentle movement can aid in recovery. Avoid heavy lifting and strenuous activities for at least six weeks post-surgery, as this can strain your healing tissues. Gradually increasing your activity level, as tolerated, can help improve circulation and promote healing.

- Follow-Up Appointments: Regular follow-ups with your healthcare provider are essential to monitor your recovery. These visits allow your doctor to assess your healing progress and address any concerns you may have.

- Signs of Complications: Be vigilant for signs of complications, such as increased bleeding, severe abdominal pain, fever, or unusual discharge. If you experience any of these symptoms, seek medical attention promptly.

In conclusion, your concerns regarding the uterine cavity and brown discharge are valid and can be explained by the normal healing process following a laparoscopic myomectomy. It’s essential to maintain open communication with your healthcare provider and attend all follow-up appointments to ensure a smooth recovery. Remember, every individual’s healing journey is unique, and it’s important to listen to your body and seek help when needed.
