Hashimoto's thyroiditis
In April of this year, when I had my cholesterol checked, my doctor also tested my thyroid function.
The results showed T4 at 1.15, TSH at 1.554, and anti-TPO at 5.57, indicating the presence of antibodies consistent with Hashimoto's thyroiditis.
The doctor advised me to supplement with the lowest dose of 50 micrograms every two days.
In October, I had a follow-up blood test, and the results were T4 at 1.15 and TSH at 1.797, both within normal ranges.
The doctor recommended continuing to supplement with thyroid hormone every two to three days.
However, my weight has decreased from 58 kg to 53.5 kg.
I would like to know if weight loss while supplementing with thyroid hormone is normal.
Should I continue taking thyroid hormone given my current situation?

Dr. Li Jian reply Internal Medicine
You may consider discontinuing the medication, and at least monitor thyroid function annually.
Thank you for your inquiry.

Hashimoto's thyroiditis is an autoimmune disorder that affects the thyroid gland, leading to hypothyroidism in many cases. In your situation, you have been diagnosed with Hashimoto's thyroiditis, as indicated by the presence of anti-TPO antibodies. Your thyroid function tests show that your T4 levels are within the normal range, while your TSH levels are also normal, suggesting that your thyroid is functioning adequately at the moment. However, the fact that you are experiencing weight loss from 58 kg to 53.5 kg raises some important considerations.
Firstly, it is essential to understand that weight changes can be influenced by various factors, including thyroid hormone levels, diet, physical activity, and other underlying health conditions. In the case of Hashimoto's thyroiditis, patients may experience fluctuations in weight due to changes in thyroid hormone levels. While hypothyroidism typically leads to weight gain, some individuals may experience weight loss, especially if they are on thyroid hormone replacement therapy, as your body adjusts to the medication.
The weight loss you are experiencing could be attributed to several factors:
1. Thyroid Hormone Replacement: When you take thyroid hormone replacement, it can increase your metabolism, leading to weight loss. However, if the dosage is too high, it may cause symptoms of hyperthyroidism, which can also lead to weight loss, increased heart rate, and anxiety.
2. Diet and Lifestyle Changes: If you have made changes to your diet or physical activity levels, these could also contribute to weight loss. It's important to evaluate your eating habits and exercise routine to see if any changes have occurred.
3. Other Health Issues: Weight loss can sometimes indicate other underlying health issues, such as gastrointestinal problems, infections, or other endocrine disorders. If you are experiencing other symptoms, such as fatigue, changes in appetite, or gastrointestinal disturbances, it would be wise to discuss these with your healthcare provider.
Regarding your current treatment plan, your doctor has recommended continuing with the thyroid hormone replacement therapy every two to three days. Given that your T4 and TSH levels are within normal ranges, it suggests that your current dosage is appropriate. However, the ongoing weight loss is a concern that should be addressed. It is crucial to communicate with your healthcare provider about your weight changes and any other symptoms you may be experiencing. They may want to monitor your thyroid function more closely or consider adjusting your medication dosage.
In summary, while weight loss can occur with thyroid hormone replacement therapy, it is essential to ensure that it is not due to other underlying health issues. Continuing your thyroid hormone replacement as prescribed is important, but you should also keep an open line of communication with your healthcare provider to monitor your symptoms and adjust your treatment plan as necessary. Regular follow-ups and blood tests will help ensure that your thyroid function remains stable and that any potential issues are addressed promptly.
