Post-Ectopic Pregnancy Surgery
Hello, doctor.
I would like to ask about my situation.
I underwent a right salpingectomy for an ectopic pregnancy on November 10.
After the surgery, I experienced a small amount of bleeding, which gradually decreased to none.
However, on November 18, I had a heavy menstrual period that has continued to today, resembling a normal menstrual flow in terms of amount and color.
Is it normal for my period to return so quickly? Thank you for your response.
L, 30~39 year old female. Ask Date: 2023/11/19
Dr. Cao Guotao reply Obstetrics and Gynecology
Hello, it should not be menstruation; it is likely postoperative bleeding.
Just monitor the situation.
Wishing you good health.

More Info
After undergoing surgery for an ectopic pregnancy, it is not uncommon for women to experience changes in their menstrual cycle. In your case, having a significant amount of bleeding following the surgery, which then tapered off, followed by what seems to be a normal menstrual period starting on November 18, can be considered within the realm of normal post-operative recovery.
When a woman has an ectopic pregnancy, especially one that requires surgical intervention such as the removal of a fallopian tube, the body undergoes significant hormonal changes. The ectopic pregnancy itself can disrupt the normal hormonal balance, and the surgery can further influence this. After the removal of the ectopic tissue and any associated bleeding, the body may take some time to readjust.
Typically, menstruation can resume anywhere from 4 to 6 weeks after surgery, depending on individual recovery and hormonal levels. In your case, the onset of what appears to be a normal menstrual period just over a week after surgery is not unusual. The bleeding you experienced post-surgery could have been a combination of surgical recovery bleeding and the onset of your menstrual cycle.
It's important to monitor the characteristics of your menstrual flow. If your bleeding is similar to your typical menstrual cycle in terms of flow, color, and duration, this is generally a good sign. However, if you experience any of the following, you should contact your healthcare provider:
1. Excessive Bleeding: Soaking through a pad or tampon every hour for several consecutive hours.
2. Severe Pain: Pain that is not manageable with over-the-counter pain relief or that feels different from your usual menstrual cramps.
3. Foul Odor: Any unusual odor accompanying the bleeding could indicate an infection.
4. Fever: A fever may suggest an infection or other complications.
In terms of your overall reproductive health, it is advisable to follow up with your healthcare provider for a post-operative check-up. They can assess your recovery and ensure that everything is healing properly. They may also want to discuss your future fertility options, especially since one fallopian tube has been removed.
In conclusion, while your experience of menstruating shortly after surgery can be normal, it is essential to remain vigilant about any concerning symptoms. Always feel free to reach out to your healthcare provider with any questions or concerns, as they can provide personalized advice based on your medical history and current health status. Your well-being is paramount, and staying informed about your body’s responses post-surgery is crucial for your recovery and future health.
