External hemorrhoids, itching around the anus?

I recently experienced external hemorrhoids, which made it difficult to sit or stand comfortably.
I went to the pharmacy and bought some ointment to apply, and after about three days, the pain subsided, and after sitz baths, the swelling reduced significantly.
However, after stopping the medication (while still continuing the sitz baths), I developed intense itching around the anal area after about three days.
The itching was so severe that I couldn't sleep and needed to continue applying the ointment to be able to rest.
Upon waking, the itching persisted, and I found myself needing to apply the ointment three times a day; without it, the itching would become unbearable, affecting my daily life.
I purchased "Hengan Hemorrhoid Ointment," and the pharmacist mentioned that itching is common with external hemorrhoids.
However, I feel that my symptoms are too severe! The itching is primarily around the anal area, rather than directly at the hemorrhoid site.
I'm wondering if this could be due to an allergic reaction or possibly from excessive cleaning at the beginning? I also tried applying petroleum jelly, but the itching continued, and I still needed to apply the ointment.
Initially, the ointment was effective… Thank you for your response.

External hemorrhoids rarely cause intense and persistent itching; it is usually due to skin irritation from excessive use of ointments.
It is recommended to discontinue all ointments, avoid over-cleaning the area around the anus (cold water rinsing is acceptable), and consider taking oral antihistamines.
If symptoms persist, a consultation with a colorectal or dermatology specialist is advised.

More Info

External hemorrhoids are swollen veins located around the anus and lower rectum, which can cause discomfort, pain, and itching. The symptoms you are experiencing, including persistent anal itching, can be quite distressing and may significantly impact your quality of life. Let's break down the situation and explore potential causes and management strategies.

Understanding External Hemorrhoids
External hemorrhoids can become inflamed due to various factors, including straining during bowel movements, prolonged sitting, or increased pressure in the pelvic area. Symptoms typically include pain, swelling, and itching around the anal region. In your case, it seems that the initial treatment with topical medication provided relief from pain and swelling, but the itching has persisted after discontinuing the medication.

Possible Causes of Persistent Itching
1. Irritation or Inflammation: Even after the pain subsides, the area may remain sensitive and inflamed, leading to itching. This can be exacerbated by factors such as moisture, sweat, or irritation from clothing.

2. Skin Conditions: Conditions like eczema or psoriasis can also cause itching around the anal area. If you have a history of skin issues, this could be a contributing factor.

3. Hygiene Practices: Over-cleaning or using harsh soaps can irritate the sensitive skin around the anus. It's essential to maintain a balance between cleanliness and irritation. Gentle cleansing with mild soap and water, followed by thorough drying, is recommended.

4. Allergic Reactions: You mentioned the possibility of an allergic reaction. Ingredients in topical medications, wipes, or even laundry detergents can cause localized allergic reactions, leading to itching.

5. Dietary Factors: Certain foods, such as spicy foods, caffeine, or alcohol, can irritate the anal area and exacerbate symptoms.

6. Infection: Fungal infections or other skin infections can also lead to itching. If the itching is severe and persistent, it may be worth considering this possibility.

Management Strategies
1. Continue Sitz Baths: Regular sitz baths can help soothe the area and reduce itching. Ensure the water is warm, not hot, and limit bath time to about 15-20 minutes.

2. Topical Treatments: While you found initial relief with the “恒安療痔妥軟膏,” it may be beneficial to consult with a healthcare provider about alternative treatments. Over-the-counter hydrocortisone cream can help reduce inflammation and itching, but it should not be used for extended periods without medical advice.

3. Avoid Irritants: Be cautious with any products you apply to the area. If you suspect an allergy, try to identify and eliminate potential irritants. Opt for fragrance-free and hypoallergenic products.

4. Dietary Adjustments: Monitor your diet for any foods that may trigger symptoms. Increasing fiber intake can help prevent straining during bowel movements, which can aggravate hemorrhoids.

5. Consult a Healthcare Provider: Given the severity and persistence of your symptoms, it may be wise to consult a healthcare provider, preferably a gastroenterologist or a proctologist. They can perform a thorough examination to rule out any underlying conditions and provide tailored treatment options.

6. Stress Management: Stress can exacerbate symptoms, so consider incorporating relaxation techniques such as yoga, meditation, or deep breathing exercises into your routine.

Conclusion
Persistent anal itching following an episode of external hemorrhoids can be frustrating and uncomfortable. While it is common for hemorrhoids to cause itching, the severity of your symptoms warrants further investigation. By addressing potential irritants, maintaining proper hygiene, and consulting with a healthcare provider, you can work towards finding relief and improving your quality of life. Remember, it’s essential to listen to your body and seek professional advice when symptoms persist or worsen.
