Elevated IgG4 levels?

Hello, doctor.
For over a year, I have been experiencing unexplained dull pain in my back, and rehabilitation has not shown significant improvement.
I have undergone X-rays and tests related to ankylosing spondylitis, all of which returned normal results.
Occasionally, I also experience abdominal bloating and discomfort.
After consulting various specialties, I was referred to a rheumatologist, who found that my IgG4 level was 465.
My current treatment involves taking rituximab, and I have been on this medication for six months.
My IgG4 level slightly decreased to 365 last month.
However, when I inquired about the cause of this condition, the doctor could not provide a clear explanation.

Additionally, over the past year, I have experienced swelling in the lymph nodes near my left clavicle four times, which felt like fluid without any hard masses.
During two of these episodes, I had abdominal bloating, and tests revealed pancreatitis.
The treatment involved antibiotics, and the swelling typically subsided within a week.

I would like to know if elevated IgG4 levels could cause unexplained swelling in certain lymph nodes.
Besides the current treatment, do you have any other recommendations or suggest other specialties I should consult? Thank you for your response.

Hello, the IgG4-related disease can cause lymphadenopathy and pancreatitis.
It is generally not possible to diagnose solely based on IgG4 levels; lymph node or organ biopsies can provide further assistance in diagnosing the disease.
It is recommended to consult a rheumatologist for IgG4-related disease, and you may also seek care from relevant specialties depending on the affected organs.
Thank you for your question.

Elevated IgG4 levels can be indicative of various underlying conditions, and understanding the causes and treatment options is crucial for effective management. IgG4 is one of the four subclasses of immunoglobulin G (IgG) and is often associated with autoimmune diseases, chronic inflammation, and certain types of cancers. In your case, the elevated IgG4 levels, along with your symptoms, warrant a comprehensive evaluation.

Causes of Elevated IgG4 Levels
1. IgG4-Related Disease: This is a condition characterized by the infiltration of IgG4-positive plasma cells into various tissues, leading to inflammation and fibrosis. It can affect multiple organs, including the pancreas, salivary glands, and lymph nodes. The symptoms you describe, such as back pain and abdominal discomfort, could be related to this condition.

2. Chronic Inflammation: Conditions such as chronic pancreatitis, which you mentioned, can lead to elevated IgG4 levels. The inflammation in the pancreas can also cause abdominal swelling and discomfort.

3. Autoimmune Disorders: Elevated IgG4 can be seen in autoimmune diseases like Sjögren's syndrome, systemic lupus erythematosus (SLE), and others. The presence of lymphadenopathy (swollen lymph nodes) could also suggest an autoimmune process.

4. Lymphoproliferative Disorders: Certain cancers, particularly those involving the lymphatic system, can lead to elevated IgG4 levels. The recurrent swelling of your left shoulder and clavicle lymph nodes may need further investigation to rule out malignancy.

Diagnostic Approach
Given your symptoms and elevated IgG4 levels, it is essential to conduct a thorough evaluation, which may include:
- Imaging Studies: Advanced imaging techniques such as MRI or CT scans can help visualize any abnormalities in the lymph nodes or other affected areas.

- Biopsy: If there are persistent lymphadenopathies, a biopsy may be necessary to determine if there is an underlying malignancy or specific inflammatory process.

- Additional Blood Tests: Tests for other autoimmune markers or inflammatory markers can provide more context to your condition.

Treatment Options
1. Medication: You mentioned being treated with azathioprine, which is an immunosuppressive medication. This can be effective in reducing inflammation associated with elevated IgG4 levels. If your IgG4 levels have decreased, it suggests that the treatment may be having a positive effect.

2. Corticosteroids: In cases of significant inflammation, corticosteroids may be prescribed to quickly reduce symptoms and inflammation. However, long-term use can have side effects, so this needs to be carefully managed.

3. Monitoring: Regular follow-up with your rheumatologist or immunologist is essential. Monitoring IgG4 levels and symptoms can help guide treatment decisions.

4. Referral to Specialists: Depending on the findings, you may need to see specialists such as a gastroenterologist for pancreatic issues or a hematologist if there are concerns about lymphoproliferative disorders.

Lifestyle and Supportive Care
In addition to medical treatment, consider the following supportive measures:
- Physical Therapy: If you have persistent back pain, physical therapy may help improve mobility and reduce discomfort.

- Dietary Adjustments: If abdominal discomfort is related to dietary factors, working with a nutritionist may provide relief.

- Stress Management: Chronic pain and illness can lead to stress, which may exacerbate symptoms. Techniques such as mindfulness, yoga, or counseling can be beneficial.

Conclusion
Elevated IgG4 levels can be associated with various conditions, and a multidisciplinary approach is often required for diagnosis and treatment. It is essential to maintain open communication with your healthcare providers, discuss your symptoms, and ensure that all potential causes are explored. Regular follow-up and monitoring will be key in managing your condition effectively. If you have further concerns or if your symptoms change, do not hesitate to seek additional medical advice.
