Interactions between oral contraceptives and ciprofloxacin?
I recently visited a doctor due to a bladder infection and forgot to inform the doctor that I am taking Yaz (drospirenone and ethinyl estradiol) as a contraceptive pill.
The doctor prescribed me the following medications: ciprofloxacin, oxybutynin extended-release, and famotidine.
I would like to ask if any of these medications will affect the contraceptive effectiveness of Yaz.

Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the following medications may interact with oral contraceptives:
1.
Antibiotics (Penicillin, Cephalosporins, Erythromycin, Sulfonamides, Bactrim, Chloramphenicol) - may reduce the gut flora that aids in absorption, thereby decreasing the effectiveness of contraceptives.
2.
Sedatives (Barbiturates) - increase the metabolism of contraceptives, reducing their effectiveness.
3.
Antiepileptic drugs (Primidone, Phenytoin, Carbamazepine) - increase the metabolism of contraceptives, reducing their effectiveness.
4.
Tuberculosis medications (Rifampicin, Isoniazid).
5.
Antidiabetic medications (Rosiglitazone).
6.
Acetaminophen and Ascorbic acid (Vitamin C) - may inhibit the metabolism of estrogen, leading to increased concentrations in the body and a higher incidence of side effects.
7.
Oral contraceptives may affect the action of anticoagulants.
8.
Smoking - increases the metabolism of contraceptives, reducing their effectiveness.
Therefore, if you need to take any of the above medications while on contraceptives, it is advisable to use additional contraceptive measures for seven days.
Additionally, long-term consumption of tea or certain herbs may also have additive or subtractive effects with oral contraceptives.
Furthermore, any medication should be taken with water and not with tea, coffee, or other beverages, as this may affect the medication's efficacy.
If the mentioned medications are taken, it is best to implement additional contraceptive measures for seven days.
Thank you! Wishing you peace and health.

More Info
When it comes to the interaction between Yaz (a combination birth control pill containing ethinyl estradiol and drospirenone) and ciprofloxacin, there are several important considerations to keep in mind. Yaz is primarily used for preventing pregnancy, regulating menstrual cycles, and managing symptoms of premenstrual dysphoric disorder (PMDD). Ciprofloxacin, on the other hand, is an antibiotic used to treat various bacterial infections, including urinary tract infections (UTIs) and bladder infections.
The concern regarding the interaction between antibiotics and hormonal contraceptives, such as Yaz, stems from the potential for antibiotics to reduce the effectiveness of birth control pills. However, it is important to note that most antibiotics, including ciprofloxacin, do not significantly affect the efficacy of hormonal contraceptives. The primary antibiotic that has been shown to have a notable interaction with birth control pills is rifampin, which is used to treat tuberculosis and certain other infections. Rifampin can decrease the effectiveness of hormonal contraceptives by increasing the metabolism of the hormones in the liver.
In the case of ciprofloxacin, studies have indicated that it does not have a clinically significant effect on the effectiveness of Yaz or other combined oral contraceptives. Therefore, if you are taking Yaz and have been prescribed ciprofloxacin for a bladder infection, you can generally continue to rely on Yaz for contraception without significant concern for reduced effectiveness.
Regarding the other medications you mentioned—voren-k (which typically contains ketorolac, a nonsteroidal anti-inflammatory drug), oxbutynin (used for overactive bladder), and famotidine (an H2 blocker used to reduce stomach acid)—none of these medications are known to interact negatively with Yaz in terms of contraceptive efficacy.
However, it is always advisable to communicate with your healthcare provider about all medications you are taking, including over-the-counter drugs and supplements, to ensure that they are aware of your complete medical history and can provide the best possible care. If you have any concerns about the interactions or side effects of these medications, or if you experience any unusual symptoms while taking them, you should reach out to your healthcare provider for further guidance.
In summary, ciprofloxacin and the other medications you mentioned are not expected to interfere with the effectiveness of Yaz. You should continue taking your birth control as prescribed, but always keep your healthcare provider informed about all medications you are using to ensure comprehensive care. If you have any further questions or concerns, do not hesitate to ask your doctor or pharmacist for clarification.
