Post-Surgery Care for Fistula Removal: Pain and Medication Concerns - Surgery

Post-fistula surgery

Hello doctor, I underwent a fistula excision surgery on January 11th.
I have been taking sitz baths four times a day, and during these baths, I experience slightly more pain than usual and notice a little bleeding.
I would like to know if this is normal.
The surgeon prescribed me with Neomycin ointment to apply twice daily, but since I take sitz baths four times a day, it washes off the ointment.
Should I reapply it after each sitz bath?

Dr. Zhang Shuhao reply Surgery

It is normal to experience slightly more pain and some bleeding during sitz baths before the wound has healed, and this should improve over time.
If sitz baths are done more frequently, the amount of ointment can be reduced, and it may be sufficient to apply it only in the morning and evening.

Post-surgery care for fistula removal is crucial for ensuring proper healing and minimizing discomfort. After undergoing a fistula excision on January 11, it is common to experience some pain and minor bleeding, especially during activities such as sitz baths. The pain you are experiencing during sitz baths, while slightly more intense than usual, can be considered a normal part of the healing process. However, it is essential to monitor the situation closely.

Sitz baths are often recommended post-surgery to promote hygiene and comfort. They can help reduce inflammation and provide relief from pain. However, if you notice that the pain is increasing significantly or if the bleeding becomes more than just a few drops, it is important to contact your healthcare provider for further evaluation. Minor bleeding can occur as the surgical site heals, but persistent or heavy bleeding may indicate a complication that needs to be addressed.

Regarding the use of the antibiotic ointment (such as Neomycin) prescribed by your surgeon, it is indeed important to maintain the effectiveness of the medication. If you are taking sitz baths four times a day, it is likely that the ointment may wash away, reducing its effectiveness. In this case, it would be advisable to reapply the ointment after each sitz bath to ensure that the surgical site remains protected from infection and promotes healing.
Here are some additional recommendations for post-surgery care:
1. Follow-Up Appointments: Ensure you attend all scheduled follow-up appointments with your surgeon. They will assess the healing process and make any necessary adjustments to your care plan.

2. Pain Management: If the pain becomes unmanageable, discuss pain management options with your doctor. They may recommend over-the-counter pain relievers or prescribe stronger medications if necessary.

3. Hygiene: Continue to maintain good hygiene around the surgical site. In addition to sitz baths, gentle cleansing with mild soap and water can help prevent infection.

4. Diet and Hydration: Eating a balanced diet and staying well-hydrated can aid in the healing process. Foods rich in vitamins and minerals, particularly vitamin C and zinc, can support tissue repair.

5. Activity Level: Avoid strenuous activities or heavy lifting until your doctor gives you the green light. Rest is essential for recovery.

6. Signs of Infection: Be vigilant for signs of infection, such as increased redness, swelling, warmth around the surgical site, or fever. If you notice any of these symptoms, contact your healthcare provider immediately.

7. Emotional Support: Surgery and recovery can be emotionally taxing. Don’t hesitate to seek support from friends, family, or mental health professionals if you are feeling overwhelmed.

In summary, while some pain and minor bleeding can be expected after fistula removal surgery, it is crucial to monitor your symptoms and maintain communication with your healthcare provider. Reapplying the antibiotic ointment after each sitz bath is advisable to ensure effective treatment. Always prioritize your health and well-being, and do not hesitate to reach out to your doctor with any concerns.
