Generalized tingling, itching, and numbness?
Hello Director Gao: I am following up on my inquiry from January 15 regarding my symptoms of generalized itching, tingling, itchy eyes, itchy ears, fungal patches, difficulty concentrating, memory decline, forgetfulness, headaches, chest pain, and chest tightness.
I first consulted an otolaryngologist who only informed me that I have nasal inflammation and prescribed oral antibiotics and medications to alleviate allergic symptoms, confirming that it is indeed allergic rhinitis.
However, I have also noticed that my nails have become brittle.
Should I see a family medicine physician for this? Since the specialist did not conduct any tests, I am unsure about the nature of my allergic symptoms.
Therefore, I am asking you again if I should visit a family medicine physician.
Thank you.

Dr. Gao Jiankai reply Internal Medicine
Hello, thank you for your inquiry.
Brittle nails are often a result of nutritional deficiencies, and this can be evaluated by a dermatologist.
If your primary care physician is unable to address your concerns, and you are also experiencing anxiety related to allergy symptoms, you may consider consulting an allergist or immunologist.
The Changhua Hospital of the Ministry of Health and Welfare is here to support you.

When experiencing systemic pain and a variety of allergic reactions, it is essential to approach the situation methodically. Your symptoms, which include widespread itching, tingling sensations, itchy eyes and ears, memory issues, headaches, and chest discomfort, suggest a complex interplay of potential allergic reactions and possibly other underlying health conditions.
Firstly, it is important to understand that allergic reactions can manifest in various ways, including skin symptoms (like itching and rashes), respiratory issues (such as nasal inflammation), and even neurological symptoms (like memory loss or concentration difficulties). The fact that you have been prescribed antibiotics and medications for allergic symptoms indicates that your healthcare provider is considering an allergic component, but it may not be the complete picture.
Given your symptoms, it would be prudent to consider a few key points:
1. Consulting a General Practitioner (GP): Since you mentioned that your current ENT specialist did not perform further investigations, it would be wise to see a GP. They can conduct a comprehensive evaluation of your symptoms, including a physical examination and possibly ordering blood tests or allergy tests. This will help in identifying any underlying conditions that may be contributing to your symptoms.
2. Allergy Testing: If allergies are suspected, your GP may refer you to an allergist for specific testing. This could include skin prick tests or blood tests to identify specific allergens that may be causing your symptoms. Understanding what you are allergic to can help in managing and avoiding triggers.
3. Nail Health: The brittleness of your nails can also be a sign of nutritional deficiencies, underlying health issues, or even a dermatological condition. It is worth discussing this with your GP as well, as they may recommend a dermatologist for further evaluation.
4. Systemic Symptoms: The combination of systemic symptoms such as chest pain and memory issues should not be overlooked. While they may be related to anxiety or allergic reactions, they could also indicate other medical conditions that require attention. It is crucial to communicate all your symptoms clearly to your healthcare provider.
5. Mental Health Considerations: Given the cognitive symptoms you described, such as memory loss and difficulty concentrating, it may also be beneficial to consider a mental health evaluation. Stress and anxiety can exacerbate physical symptoms and lead to a cycle of discomfort that can be challenging to break.
6. Follow-Up: After your initial consultation with a GP, ensure that you follow up on any referrals or tests recommended. Keeping a symptom diary can also be helpful in tracking your symptoms and identifying patterns or triggers.
In summary, while your current treatment for allergic rhinitis is a good starting point, the complexity of your symptoms warrants a more thorough investigation. A visit to a general practitioner is a logical next step, as they can coordinate your care and ensure that all aspects of your health are being addressed. Remember, it is essential to advocate for your health and seek further evaluations if you feel your concerns are not being fully addressed. Your health is paramount, and getting to the root of your symptoms will help you find relief and improve your quality of life.
