Lower Back Pain After an Elbow Impact: When to Seek Help - Family Medicine

Right lower back struck by an elbow?

Hello Doctor: Today at the market, due to the crowd, I was struck hard in the lower right back (near the lower ribs) by the elbow of a middle-aged woman.
Currently, my entire right back intermittently aches, with a pain level of about 6.
I have been applying ice continuously.
I have two questions: 1.
Is it possible that I have sustained a rib injury? If the pain persists for a long time without improvement, should I seek medical attention? 2.
Is there a possibility of kidney injury? (Currently, I have no blood in my urine, but due to personal health reasons, I will undergo a neck CT scan in two weeks that requires contrast dye.
Should I inform the hospital about this incident?)

Dr. Xiao Yongxun reply Family Medicine

Dear Miss QQ: 1.
Everyone has a different tolerance for pain; if you suspect a rib fracture or injury, it is recommended that you go directly to a surgical outpatient clinic at a regional hospital or higher for evaluation, and undergo a chest X-ray if necessary.
2.
Based on your current description, there is no evidence of kidney injury.
If you have further concerns, it is advisable to visit a surgical outpatient clinic at a regional hospital or higher for evaluation, and undergo urine or imaging tests if necessary.
3.
Any newly occurring clinical symptoms or evidence should be reported to your attending physician; therefore, please contact the physician at your original hospital for further assistance.

Experiencing lower back pain after an elbow impact can be concerning, especially when the pain persists. Based on your description, it is essential to consider the potential injuries that could have occurred due to the impact. Let's address your questions systematically.

1. Possibility of Rib Injury: Given that you were struck forcefully in the lower back area near the ribs, there is a possibility of rib injury, including fractures or contusions. Rib injuries can manifest as localized pain that worsens with movement, deep breathing, or coughing. The pain level you describe (around 6 on a scale of 10) suggests that the injury may be significant enough to warrant further evaluation. If the pain persists for more than a few days, worsens, or is accompanied by symptoms such as difficulty breathing, swelling, or bruising, it is advisable to seek medical attention. A healthcare provider may perform imaging studies, such as X-rays, to assess for any fractures or other injuries.

2. Possibility of Kidney Injury: While the impact was on your lower back, which is near the kidneys, the absence of blood in your urine is a reassuring sign that a significant kidney injury is less likely. However, kidney injuries can sometimes occur without immediate symptoms. If you experience any new symptoms, such as flank pain (pain on the side of your body), changes in urination, or abdominal pain, it would be prudent to seek medical attention. Regarding your upcoming CT scan of the neck, it is essential to inform the medical staff about your recent injury, as they may need to consider this in their assessment and imaging interpretation.

Additional Considerations
- Ice Therapy: Continuing to apply ice to the affected area is a good initial treatment strategy. Ice can help reduce inflammation and numb the pain. It is typically recommended to apply ice for 15-20 minutes every hour as needed during the first 48 hours post-injury.

- Rest and Activity Modification: Avoid activities that exacerbate the pain, and allow your body time to heal. Gentle movements and stretching may help, but listen to your body and avoid pushing through pain.

- Over-the-Counter Pain Relief: If you have not already, consider using over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen, which can help alleviate pain and reduce inflammation. Ensure you follow the recommended dosages and consult with a healthcare provider if you have any underlying health conditions or are taking other medications.

- Monitoring Symptoms: Keep a close watch on your symptoms. If you notice any changes, such as increased pain, swelling, or new symptoms, it is crucial to seek medical attention promptly.

- Follow-Up: If your pain does not improve within a week or if it worsens, it would be wise to follow up with a healthcare provider. They may recommend further imaging or a referral to a specialist, such as an orthopedic doctor or a pain management specialist, depending on the findings.

In summary, while rib and kidney injuries are possibilities, the absence of blood in your urine is a positive sign. However, persistent pain warrants further evaluation. Always prioritize your health and seek medical advice when in doubt.
