How long after receiving the long-acting injection of Medroxyprogesterone Acetate (commonly known as Depo-Provera) will menstruation return?

I underwent laparoscopic myomectomy on September 27, 2023.
The day after the surgery, I received a long-acting injection of Leuprolide, and the doctor mentioned that the medication would be effective for three months.
During my follow-up appointment on February 5, 2024, my blood test results were as follows: Hemoglobin: 13.3, Estradiol: 9.4, CA-125: 13.0, FSH: 6.6.
The doctor said to wait for my menstrual period to arrive.
Dr.
Tsao, can my blood test results indicate when I might expect my period? Should I take calcium supplements? Thank you, doctor!

After undergoing a laparoscopic myomectomy and receiving a long-acting leuprolide injection, it is understandable to have questions about the return of your menstrual cycle. Leuprolide is a GnRH agonist that is often used to manage conditions like uterine fibroids by suppressing estrogen production. This suppression can lead to a temporary cessation of menstruation, which is expected and typically reversible once the medication is discontinued.

Based on your timeline, you received the leuprolide injection on September 28, 2023, and the medication is designed to last for approximately three months. This means that the effects of the injection will likely continue until late December 2023 or early January 2024. Therefore, it is reasonable to expect that your menstrual cycle may not return until after this period, as your body will need time to recover from the hormonal suppression caused by the medication.

Regarding your blood test results from February 5, 2024, the levels of estradiol (9.4 pg/mL) are quite low, which is consistent with the effects of leuprolide. Estradiol is a form of estrogen, and low levels indicate that your body is still under the influence of the medication. The FSH (follicle-stimulating hormone) level of 6.6 mIU/mL is within the normal range, suggesting that your pituitary gland is functioning properly and trying to stimulate the ovaries. However, without adequate estrogen production, ovulation and menstruation are unlikely to occur.

As for your question about whether you need to take calcium supplements, this is a consideration worth discussing with your healthcare provider. Leuprolide can lead to decreased bone density over time due to lower estrogen levels, which is particularly important for women. If you have risk factors for osteoporosis or if your diet is low in calcium, your doctor may recommend calcium and vitamin D supplementation to support bone health.

In summary, your menstrual cycle is likely to return after the effects of the leuprolide injection wear off, which may take several months. Your blood test results indicate that your body is still responding to the medication, and it is essential to give it time. If you have concerns about your menstrual cycle or bone health, it is advisable to follow up with your healthcare provider for personalized recommendations and possible adjustments to your treatment plan. They can provide guidance on when to expect your period to return and whether any additional supplements are necessary for your health.
