Tight testicles?

I have been experiencing a tight sensation in my left testicle for about a month.
This feeling extends into my lower abdomen and feels like muscle soreness, becoming more pronounced when lying down and during erections.
When lying down, I notice that my left testicle is positioned higher than my right, and it feels like a muscle is pulling on the testicle, but this sensation does not occur when sitting.
Upon examination, there are no prominent varicose veins on the testicles, so it doesn't seem to be a case of varicocele.
What could be the cause of this? How should it be treated? Additionally, I have an issue with penile adhesions, particularly around the coronal sulcus, which causes bleeding when I try to forcibly separate them.
Do I need a circumcision for this? Will it affect my development?

Dr. Du Shixi reply Urology

Hello: Avoid vigorous movements.
If there is adhesion of the foreskin, you can soak it in warm water and gently separate it a little each day; the longer you do this, the better.
If it doesn't improve and there is still bleeding, you should seek medical attention.
Circumcision is not always necessary, and it should not affect development.
Wishing you good health.

When it comes to testicular tightness and related urological concerns, it's essential to understand that various factors can contribute to the sensations you're experiencing. The tightness in your left testicle, which has persisted for about a month, could be attributed to several potential causes, including muscular strain, nerve irritation, or even issues related to the testicle itself.

Firstly, the sensation of tightness that extends to your lower abdomen may suggest a muscular or nerve-related issue. If you have been engaging in activities that involve heavy lifting or straining, this could lead to muscle fatigue or strain in the pelvic area, which might manifest as discomfort or tightness in the testicles. Additionally, nerve irritation in the pelvic region can also lead to referred sensations in the testicles.

The observation that your left testicle appears higher than the right when lying down could be due to a variety of anatomical or physiological reasons. In some cases, the position of the testicles can change based on body posture, and this is typically normal. However, if you notice a significant difference in size, shape, or position, it may warrant further investigation.

Regarding the absence of visible varicose veins, it is reassuring that you do not see signs of a varicocele, which is a common cause of testicular discomfort. However, other conditions, such as epididymitis (inflammation of the epididymis) or testicular torsion, should be considered, especially if you experience any additional symptoms such as swelling, redness, or severe pain.

As for your concern about the adhesion of the foreskin, this is a condition known as phimosis, which can occur when the foreskin cannot be easily retracted over the glans (the head of the penis). If the adhesions are causing pain, bleeding, or difficulty in hygiene, it may be advisable to consult a urologist. In some cases, a circumcision may be recommended to alleviate these issues, but this decision should be made based on a thorough evaluation by a healthcare professional.

In terms of treatment, if the tightness is muscular, conservative measures such as rest, ice application, and over-the-counter anti-inflammatory medications may help alleviate discomfort. If the symptoms persist or worsen, it is crucial to seek medical attention. A urologist can perform a physical examination, possibly including an ultrasound, to rule out any underlying conditions that may require treatment.

In summary, while the sensations you are experiencing may not immediately indicate a severe issue, it is essential to monitor your symptoms closely. If you experience any worsening of your condition, such as increased pain, swelling, or changes in urination, it is crucial to seek medical attention promptly. A healthcare professional can provide a comprehensive evaluation and recommend appropriate treatment based on your specific situation.

It sounds like there are no major issues; it may be caused by changes in emotions and the sympathetic nervous system. If there are still concerns, it is advisable to see a urologist for an in-person examination and to discuss it with the physician.
