Balanitis - Dalacin Cream
Hello Doctor, around March I had balanitis (with red spots and itching).
I tested for STIs and everything was normal.
Since I was abroad at the time, a doctor there prescribed Daktarin cream for me to apply.
After about a month, my symptoms improved, but the glans still appeared somewhat red.
Recently, after returning to Taiwan, I consulted a doctor who prescribed me Dalacin cream and Fungizone capsules.
After applying the Dalacin cream for three days, I noticed a significant increase in redness on the glans, and it seems to be spreading more each day (previously it was a light red, but now the color has deepened and the area is gradually expanding, although there is no discharge and it is not painful or itchy).
Should I continue using the cream in this situation? Also, could my condition possibly be bacterial balanitis rather than fungal balanitis, which might explain the minimal effect of the Dalacin cream?

Hello: If there is increased redness, it may indicate poor treatment response, please return for a follow-up.
Additionally, pay attention to cleanliness and dryness, wear loose-fitting underwear, and minimize the use of spicy and irritating foods.
Alcohol should be avoided, and practice safe sex.
There is a possibility of bacterial or fungal infections.
Wishing you good health.

More Info
Balanitis, or inflammation of the glans penis, can be caused by various factors, including infections (both fungal and bacterial), irritants, and underlying medical conditions. Given your experience with symptoms such as redness and itching, it's understandable that you're concerned about the effectiveness of your current treatment and whether the condition might be bacterial rather than fungal.
From your description, it seems that you initially received Daktarin, which is an antifungal medication, and experienced some improvement. However, upon returning to Taiwan and starting treatment with the topical cream (達來乳膏) and oral medication (膚黴克膠囊), you noticed an increase in redness and the area affected. This could indicate a few possibilities.
1. Fungal vs. Bacterial Infection: While Daktarin targets fungal infections, if your condition is primarily bacterial, the antifungal treatment may not be effective. Bacterial balanitis can present similarly to fungal infections but may require different treatment, typically involving antibiotics. The absence of discharge and pain does not rule out a bacterial infection, as some cases can be less symptomatic.
2. Irritation or Allergic Reaction: The worsening of redness after applying the topical cream could also suggest an irritation or allergic reaction to the medication itself. Some topical treatments can cause contact dermatitis, leading to increased redness and discomfort. If the area continues to worsen, it may be wise to discontinue the use of the cream and consult your healthcare provider.
3. Underlying Conditions: Conditions such as diabetes or skin disorders (like psoriasis or eczema) can predispose individuals to balanitis. If you have any underlying health issues, they may need to be addressed to effectively treat the inflammation.
Given your current situation, it is advisable to:
- Stop Using the Cream: If you notice that the redness is increasing and the condition is not improving, it may be best to stop using the topical cream and consult your doctor. Continuing to use a treatment that seems to exacerbate the condition could lead to further irritation.
- Consult a Healthcare Provider: It’s crucial to follow up with a healthcare provider who can reassess your condition. They may recommend a culture or swab of the affected area to determine the exact cause of the inflammation, whether it be fungal, bacterial, or another etiology.
- Consider Alternative Treatments: If a bacterial infection is suspected, your doctor may prescribe an appropriate antibiotic. If the issue is fungal, they might suggest a different antifungal treatment or a combination therapy.
- Maintain Hygiene: Keeping the area clean and dry is essential. Avoid irritants such as harsh soaps or chemicals, and consider wearing loose-fitting underwear to reduce friction and moisture buildup.
In summary, your situation warrants careful evaluation by a healthcare professional to determine the underlying cause of your symptoms and to adjust your treatment plan accordingly. It’s important to address these concerns promptly to avoid complications and ensure effective management of your condition.
