The entire upper body often experiences cramping pain?
Since last June, I have been experiencing intermittent sharp pain below my chest near the ribs for a year.
Accompanied by skin itching, I initially thought it was a liver issue, and the pain lasted for a month.
The pain occurs sporadically throughout the day but is manageable and not severe.
After a month, it improved, but since then, I occasionally experience similar sharp pain in the ribs under my armpit, along with shoulder pain.
Sometimes the pain in my armpit radiates to my shoulder.
Recently, I experienced a different kind of pain; while sleeping, I suddenly had severe pain near the "Gao Huang" area (the area near the lower back).
The pain was so intense that I couldn't sleep that night, and the next day I was scared and kept stretching my back.
Eventually, the pain gradually subsided, but any chest expansion movement would make my back feel comfortable.
After a few days, the pain in the "Gao Huang" area disappeared, but it returned as a combination of pain in both shoulders, ribs, armpits, and shoulders, moving around.
Now it feels more like soreness.
I am unsure about the possible causes of this condition and which specialist I should consult.

It is difficult to make a judgment based solely on the text description.
I recommend that you seek evaluation from an orthopedic specialist, a hepatobiliary and gastroenterology specialist, or a family medicine physician.
Thank you.

Understanding intermittent rib and shoulder pain can be quite complex, especially when it has persisted for an extended period, as in your case. Based on your description, it seems you have experienced a variety of symptoms, including rib pain, shoulder pain, and discomfort in the axillary region, which can be indicative of several underlying issues.
Firstly, the intermittent nature of your rib and shoulder pain could suggest a musculoskeletal origin, such as muscle strain or inflammation of the joints, particularly if the pain is exacerbated by certain movements or positions. Conditions like costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, could lead to sharp, localized pain that may radiate to the shoulder or back. Additionally, issues related to the rotator cuff or shoulder joint could also manifest as referred pain in the rib area.
The fact that you have experienced episodes of itching along with your rib pain raises the possibility of a dermatological condition or even a liver-related issue, as you initially suspected. However, it is essential to note that while liver problems can cause referred pain in the right upper quadrant, they typically present with additional symptoms such as jaundice, changes in urine or stool color, and systemic symptoms like fatigue or weight loss. If these symptoms are absent, it may be less likely that your liver is the primary issue.
The sudden onset of severe pain near the "膏肓" (which refers to a specific acupuncture point located in the back) during sleep is concerning. This could indicate a more acute condition, such as a muscle spasm, nerve impingement, or even a herniated disc in the thoracic spine, which can cause referred pain to the ribs and shoulders. If the pain was severe enough to disrupt your sleep, it warrants further investigation.
Given the complexity of your symptoms, I recommend the following steps:
1. Consult a Healthcare Provider: It would be wise to see a primary care physician or a specialist in musculoskeletal disorders, such as an orthopedist or a rheumatologist. They can perform a thorough physical examination and may recommend imaging studies like X-rays or MRIs to rule out structural issues.
2. Pain Management: In the meantime, over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help manage pain and inflammation. However, ensure you follow the recommended dosage and consult with a healthcare provider if you have any underlying health conditions or are taking other medications.
3. Physical Therapy: If musculoskeletal issues are confirmed, physical therapy may be beneficial. A physical therapist can provide exercises to strengthen the shoulder and back muscles, improve flexibility, and reduce pain.
4. Monitor Symptoms: Keep a detailed record of your symptoms, including when they occur, their intensity, and any associated activities. This information can be invaluable for your healthcare provider in diagnosing the issue.
5. Consider Other Specialists: If your symptoms persist despite initial treatment, consider consulting a neurologist to evaluate for any nerve-related issues, especially given the radiating nature of your pain.
In summary, while your symptoms may stem from a variety of causes, it is crucial to seek medical attention to determine the underlying issue accurately. Early intervention can prevent potential complications and improve your quality of life.
