Is this really... not acceptable?
Hello doctor, when I engage in sexual activity with my girlfriend, sometimes during intense moments or when my breathing quickens, my penis inexplicably becomes flaccid before ejaculation.
At times, I can perform normally, and the duration is usually around 5 minutes.
Previously, this situation occurred only occasionally, but in the last 2 to 3 months, it happens about once every two times on average, which has been making me feel increasingly powerless.
I also experience a bit of frequent urination, feeling the need to go about once an hour, and the amount each time isn't much.
Over a year ago, I consulted a doctor who mentioned it might just be prostatitis and prescribed medication, but it didn't seem to help.
However, in the past six months, my urine has been consistently foamy.
I often read in newspapers and magazines that foamy urine usually indicates kidney problems, and I'm unsure if the foam in my urine is related to sexual activity.
Doctor, could you explain why this is happening? I'm only 23 years old...
is this really a problem?

Mr.
Gu: Hello, I apologize for the delayed response due to my busy schedule.
Regarding your questions, my opinions are as follows:
1.
Concerning sexual function: Typically, sexual activity is a mutual endeavor, and I have always emphasized this concept.
In other words, sex is not solely the responsibility of either the male or female partner; it requires cooperation and learning from both sides.
Your situation appears to stem from a lack of experience.
As you accumulate experience in sexual activity, you will discover what works best for both of you.
2.
Regarding the color and appearance of urine: Many people mistakenly believe that certain colors in urine indicate specific diseases; this is an incorrect notion.
If it were that simple, there would be no need for laboratory testing.
Based on experience, the following conditions generally indicate a problem: red urine, foul odor, and significant sediment.
As for the bubbles you mentioned, they are clinically less relevant.
If you are concerned, you can visit a nearby medical facility for a urine test.
3.
The issue of frequent urination involves a broader range of factors, including anxiety, infections, increased fluid intake, and kidney stones, among others.
Additionally, some internal medical conditions, such as diabetes and diabetes insipidus, could also be potential causes.
Further examination may be necessary to determine the underlying issue.
Sincerely,

Erectile issues and urinary symptoms in young men can be concerning and often lead to anxiety, especially when they occur during intimate moments. Based on your description, it seems you are experiencing a combination of erectile dysfunction (ED) and urinary symptoms, which may be interconnected.
Firstly, it's important to understand that erectile dysfunction can occur for various reasons, including psychological factors, physical health issues, and lifestyle choices. In your case, the fact that this issue has become more frequent over the past few months could indicate an underlying concern that warrants further investigation. Stress, anxiety, and performance pressure during sexual activity can contribute to temporary erectile issues. It’s not uncommon for young men to experience fluctuations in erectile function, especially during moments of heightened excitement or anxiety.
Your mention of frequent urination and the sensation of needing to urinate every hour, along with the small volume of urine produced, could suggest a condition affecting your urinary tract or bladder. The presence of bubbles in your urine can be indicative of several factors, including dehydration, proteinuria (excess protein in urine), or even a urinary tract infection (UTI). While some bubbles can be normal, persistent foamy urine, especially if accompanied by other symptoms, should be evaluated by a healthcare professional.
The previous diagnosis of prostatitis (inflammation of the prostate) could be relevant here. Prostatitis can lead to urinary symptoms such as increased frequency, urgency, and discomfort. It can also affect erectile function, as the prostate plays a role in sexual health. If you were treated for prostatitis but did not see improvement, it may be worth revisiting this diagnosis with your healthcare provider. Chronic prostatitis can sometimes be difficult to treat and may require a multifaceted approach, including lifestyle changes, medication, and possibly physical therapy.
In terms of lifestyle factors, consider evaluating your daily habits. Regular exercise, a balanced diet, and adequate hydration can significantly impact both urinary and erectile function. Reducing stress through mindfulness practices or counseling may also help alleviate performance anxiety during sexual encounters.
It’s crucial to communicate openly with your healthcare provider about all your symptoms, including the erectile issues, urinary frequency, and the presence of bubbles in your urine. They may recommend further tests, such as a urinalysis, ultrasound, or even a referral to a urologist for a more specialized evaluation.
In summary, while occasional erectile issues can be normal, persistent symptoms warrant a thorough evaluation to rule out underlying health concerns. Addressing both the psychological and physical aspects of your symptoms will be key to finding a resolution. Remember, seeking help is a positive step towards understanding and improving your sexual and urinary health.
