Nail abnormalities?
Hello Doctor: Recently, I have noticed small white spots appearing on my fingernails.
One of my fingernails has two prominent white lines, approximately 0.5 centimeters long, which seem to be formed by the aggregation of these white spots.
Previously, only one fingernail would show white spots, but now almost every nail has subtle white spots.
They appear to resemble nail trauma caused by external force, but they are indeed emerging from within.
Since I have one nail affected by onychomycosis, I have to take oral medication daily.
I would like to know if the white spots could be a side effect of the medication or if they are a sign of an issue with a part of my body or an organ.
Thank you!

Online diagnosis is not possible; you should visit the dermatology clinic to pick up your antifungal medication for nail fungus and inquire about it at the same time.

The appearance of white spots or lines on the nails, medically referred to as leukonychia, can be concerning for many individuals. It is essential to understand the potential causes and implications of these changes in your nails, especially in the context of your ongoing treatment for a fungal infection (onychomycosis) with oral medication.
Leukonychia can manifest in various forms, including small white spots, lines, or even complete whitening of the nail. The most common causes of white spots on nails include:
1. Trauma or Injury: Often, white spots are a result of minor trauma to the nail matrix (the area where the nail grows). This could be from banging your nails against a hard surface or even from aggressive manicuring. The spots typically grow out with the nail over time.
2. Allergic Reactions: Allergies to nail products, such as nail polish or nail hardeners, can also lead to white spots. If you have recently changed your nail care products, this could be a potential cause.
3. Nutritional Deficiencies: Deficiencies in certain vitamins and minerals, particularly zinc and calcium, can lead to changes in nail appearance, including white spots.
4. Systemic Health Issues: In some cases, white spots on nails can indicate underlying health problems, such as liver disease, kidney disease, or other systemic conditions. However, these are less common and usually accompanied by other symptoms.
5. Fungal Infections: Given your history of onychomycosis, it's important to consider that fungal infections can also cause changes in nail appearance. While fungal infections typically lead to discoloration and thickening of the nail, they can sometimes present with white spots or lines.
6. Medication Side Effects: Certain medications can have side effects that affect nail growth and appearance. While it is less common for oral antifungal medications to cause leukonychia, it is not impossible. If you suspect that your medication may be contributing to the changes in your nails, it is crucial to discuss this with your healthcare provider.
In your case, since you have noticed the white spots appearing on multiple nails and have a history of gray nail fungus, it would be prudent to consult with a dermatologist or a healthcare professional specializing in nail disorders. They can perform a thorough examination, possibly including a nail biopsy or fungal culture, to determine the exact cause of the white spots.
In the meantime, here are some general recommendations for nail care:
- Avoid Trauma: Be gentle with your nails to prevent further injury. Avoid using your nails as tools.
- Moisturize: Keep your nails and cuticles moisturized to prevent dryness and brittleness.
- Balanced Diet: Ensure you are consuming a balanced diet rich in vitamins and minerals, particularly those known to support nail health, such as biotin, zinc, and protein.
- Limit Chemical Exposure: If you suspect an allergic reaction, consider reducing the use of nail products and observe if there is any improvement.
In conclusion, while white spots on nails can be benign and often resolve on their own, they can also indicate underlying issues that may require medical attention. Given your ongoing treatment for a fungal infection, it is essential to keep an open line of communication with your healthcare provider to address any concerns and ensure optimal nail health.
