Itching of the glans and inner foreskin?

Hello Doctor: I would like to ask about itching, particularly at the base of the glans and inside the foreskin.
It has been a week and it hasn't improved.
Initially, I felt itchy all day, but now it only occurs at night while sleeping.
However, when I shower and expose the area, it becomes very itchy.
Recently, I've noticed that the glans has become dry and is peeling, but there is only itching and no pain.
Could this be an allergy or is there another issue? Which specialist should I see?

A-Dai, 20~29 year old female. Ask Date: 2005/01/12

Dr. Chen Yiying reply Dermatology

Hello: In response to your question...
Your condition may be due to balanoposthitis, which can often result from excessive washing or improper handling.
You should avoid using soap for cleaning to prevent irritation.
If you experience dryness or itching, you can apply petroleum jelly or an oily eye ointment.
If there is no improvement, you may seek consultation from a dermatologist or urologist.

More Info

Itching in the glans and inner foreskin can be a distressing symptom, and understanding its potential causes is crucial for effective management. Based on your description, it seems you have been experiencing persistent itching, dryness, and peeling in the area, which has not improved over the past week. While I cannot provide a definitive diagnosis, I can outline some common causes and suggest appropriate steps to take.

Possible Causes of Itching in the Glans and Inner Foreskin
1. Allergic Reactions: Allergies to soaps, lotions, or even laundry detergents can lead to irritation and itching. If you have recently changed any products you use in that area, consider reverting to your previous products to see if the symptoms improve.

2. Infections: Fungal infections, such as candidiasis (yeast infection), are common causes of itching and can lead to dryness and peeling. Bacterial infections can also occur, especially if there is any irritation or injury to the skin.

3. Dermatitis: Contact dermatitis, which is an inflammatory reaction of the skin due to irritants or allergens, can cause itching and dryness. This can be due to personal hygiene products, sexual lubricants, or even certain fabrics.

4. Poor Hygiene: Inadequate cleaning can lead to the accumulation of smegma, which can irritate the skin and cause itching. Conversely, excessive washing can strip the skin of its natural oils, leading to dryness.

5. Skin Conditions: Conditions such as psoriasis or eczema can also manifest in the genital area, leading to symptoms like itching, dryness, and peeling.

6. Sexually Transmitted Infections (STIs): Some STIs can cause itching and irritation in the genital area. If you have had unprotected sexual contact, it may be worth considering this possibility.

Recommended Actions
1. Consult a Healthcare Provider: Given that your symptoms have persisted for a week and are causing discomfort, it is advisable to see a healthcare provider. A dermatologist or a urologist would be appropriate specialists to consult for this issue. They can perform a physical examination and possibly conduct tests to determine the underlying cause.

2. Avoid Irritants: In the meantime, avoid using any new soaps, lotions, or other products in the genital area. Stick to mild, unscented products to minimize irritation.

3. Maintain Hygiene: Ensure that you are cleaning the area gently but thoroughly. You may want to retract the foreskin (if applicable) during washing to clean underneath, but be sure to do so gently to avoid irritation.

4. Moisturize: If the area is dry, consider using a gentle, fragrance-free moisturizer to help alleviate dryness. However, avoid applying anything that could potentially irritate the skin further.

5. Monitor Symptoms: Keep track of any changes in your symptoms, including the presence of any discharge, changes in color, or additional symptoms like pain or swelling, which could indicate a more serious condition.

Conclusion
In summary, the itching you are experiencing in the glans and inner foreskin could be due to a variety of causes, including allergic reactions, infections, dermatitis, or skin conditions. It is essential to consult a healthcare provider for an accurate diagnosis and appropriate treatment. In the meantime, maintaining good hygiene, avoiding irritants, and monitoring your symptoms can help manage discomfort. Remember, early intervention can often lead to better outcomes, so do not hesitate to seek medical advice.
