Foreskin
I have small, white, raised bumps of varying sizes on my foreskin after retracting it.
They are not painful or itchy.
Could this be genital warts? If not, what could it be?

Dr. Jiang Zheen reply Dermatology
It should not be a cauliflower, but rather a common normal tissue known as a microvascular fibroma, which is not a cause for concern.

Understanding white bumps under the foreskin can be concerning, especially when they appear suddenly or change in size or shape. The presence of white bumps or spots under the foreskin can be attributed to several benign conditions, and it is essential to differentiate between them to alleviate concerns and determine if any treatment is necessary.
One common cause of white bumps under the foreskin is Pearly Penile Papules. These are small, dome-shaped, white or skin-colored bumps that typically appear around the corona of the glans (the head of the penis). They are completely harmless and are considered a normal anatomical variation. Pearly penile papules are not associated with any disease or infection and do not require treatment unless they cause psychological distress.
Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the shaft of the penis or the scrotum. These spots are sebaceous glands that are not associated with hair follicles and are also a normal variant. Like pearly penile papules, Fordyce spots are benign and do not require treatment.
If the white bumps are larger, irregular, or accompanied by other symptoms such as pain, itching, or discharge, it may indicate a different condition. One such condition is Balanitis, which is inflammation of the glans and can be caused by infections (bacterial or fungal), poor hygiene, or skin conditions. Symptoms may include redness, swelling, and discomfort. Treatment typically involves maintaining good hygiene and may require antifungal or antibacterial medications if an infection is present.
Another concern could be Genital Warts, which are caused by certain strains of the human papillomavirus (HPV). These warts can appear as small, flesh-colored or white bumps and may be raised or flat. They can be itchy or uncomfortable and may require treatment through topical medications, cryotherapy, or other methods.
In rare cases, white bumps could be indicative of Lichen Sclerosus, a chronic skin condition that can affect the genital area. This condition may cause white patches, itching, and discomfort. It is essential to consult a healthcare provider for an accurate diagnosis and appropriate management.
Given your description of the bumps being painless and not causing any discomfort, it is likely that they are benign, such as pearly penile papules or Fordyce spots. However, it is crucial to consult a healthcare professional, preferably a urologist or dermatologist, for a thorough examination and accurate diagnosis. They can provide reassurance, rule out any serious conditions, and discuss any necessary treatment options if needed.
In summary, while white bumps under the foreskin can be alarming, many cases are benign and do not require treatment. However, if there are any changes in the appearance of the bumps, or if you experience any discomfort, it is advisable to seek medical advice to ensure proper care and peace of mind.
