Urgent: Hand Nerve Pain

Hello, Dr.
Yeh: For the past 10 days, I have been experiencing pain in the nerves of my hands upon waking up, making it difficult to help my child get dressed.
The pain started in my upper arm, then two days later it extended to my elbow, and a few days later it affected my entire hand, including my fingers.
However, this condition improves slightly by the afternoon.
Could you please explain what might be causing this pain? Thank you for your response.

Juan, 30~39 year old female. Ask Date: 2004/11/22

Dr. Ye Boshou reply Neurology

Is there a possibility of a nerve plexus lesion? It is advisable to see a neurologist for an evaluation.

Morning hand nerve pain can be a distressing condition, especially when it interferes with daily activities such as dressing a child. Based on the symptoms you've described, including pain that starts in the upper arm and radiates down to the elbow and entire hand, there are several potential causes to consider.

One common cause of morning hand pain is nerve compression, which can occur due to various reasons. For instance, sleeping in an awkward position may lead to pressure on the nerves in the arm or neck, resulting in pain and discomfort upon waking. This is often referred to as "positional neuropathy." If you are experiencing symptoms that improve throughout the day, it may suggest that the nerve is being relieved from compression as you move and use your arm.

Another possibility is the presence of a condition known as carpal tunnel syndrome, which is characterized by compression of the median nerve as it passes through the wrist. Symptoms typically include pain, numbness, and tingling in the fingers, particularly the thumb, index, and middle fingers. However, in your case, the pain seems to be more widespread, which may indicate a different underlying issue.

Additionally, conditions such as cervical radiculopathy, where a nerve in the neck is compressed or irritated, can lead to pain that radiates down the arm. This could explain the progression of pain from the upper arm to the elbow and hand. Other potential causes include inflammatory conditions like rheumatoid arthritis, which can cause joint pain and stiffness, particularly in the morning.

Given the complexity of your symptoms, it is advisable to consult a neurologist or an orthopedic specialist who can conduct a thorough examination. They may recommend imaging studies, such as an MRI or X-rays, to assess for any structural issues in the cervical spine or shoulder region. Electromyography (EMG) and nerve conduction studies can also be useful in determining if there is any nerve damage or compression.

In terms of treatment, management strategies may include physical therapy to improve range of motion and strengthen the muscles around the affected area. Ergonomic adjustments to your sleeping position or using supportive pillows may help alleviate pressure on the nerves during sleep. Non-steroidal anti-inflammatory drugs (NSAIDs) can also be effective in reducing pain and inflammation.

In conclusion, morning hand nerve pain can arise from various causes, including nerve compression, cervical issues, or inflammatory conditions. A comprehensive evaluation by a healthcare professional is essential to determine the exact cause and appropriate treatment plan. Early intervention can help prevent the progression of symptoms and improve your quality of life.
