What disease is it?

I have been feeling a bit depressed, and I have experienced abdominal bloating for over three months.
I have abdominal sounds, occasional pain in the upper left abdomen, and my stools are very loose, often just small pieces.
This situation tends to recur frequently; although it is temporarily not present now, I do experience constipation, which also comes and goes.
Despite regularly consuming vegetables and fruits, there has been no improvement, and I often pass gas.
I have seen many doctors, but the condition persists.

Hello, based on your description of symptoms, it seems more like a gastrointestinal dysfunction, with occasional constipation, abdominal pain, and sometimes diarrhea, resembling the manifestations of irritable bowel syndrome (IBS).
There are many potential causes, including diet, stress from daily life, and emotional pressure.
It is generally recommended to increase your intake of fruits, vegetables, and fiber, reduce stress, and consider medication treatment.
You may refer to websites related to irritable bowel syndrome to gain more understanding of this condition.
However, if your symptoms worsen or change, it may be necessary to undergo a colonoscopy to rule out any potential bowel diseases.
Sincerely, Dr.
Chen Zhi-Ping, Internal Medicine.

Chronic digestive issues can be incredibly frustrating and can significantly impact your quality of life. Based on your description, it sounds like you are experiencing a combination of symptoms, including bloating, abdominal pain, irregular bowel movements, and excessive gas. These symptoms have persisted for over three months, which suggests that they may be part of a chronic condition rather than a temporary issue.

Firstly, it’s important to understand that the digestive system is complex and can be influenced by a variety of factors, including diet, stress, and underlying medical conditions. The symptoms you are experiencing could be indicative of several potential issues, such as Irritable Bowel Syndrome (IBS), food intolerances, or even gastrointestinal infections. IBS is particularly common and is characterized by symptoms like abdominal pain, bloating, and changes in bowel habits, including diarrhea and constipation.

Given that you have seen multiple doctors without finding a resolution, it may be beneficial to seek a gastroenterologist's opinion if you haven't already. They can perform specific tests to rule out conditions such as celiac disease, inflammatory bowel disease (IBD), or infections that might be causing your symptoms. Additionally, they may recommend tests like a colonoscopy or imaging studies to get a clearer picture of what is happening in your digestive tract.

In the meantime, there are several strategies you can implement to help manage your symptoms:
1. Dietary Modifications: Keeping a food diary can be helpful to identify any potential food triggers. Some people with digestive issues find relief by following a low-FODMAP diet, which involves reducing certain types of carbohydrates that can ferment in the gut and cause symptoms. Foods high in FODMAPs include certain fruits, vegetables, dairy products, and grains.

2. Hydration and Fiber: Ensure you are drinking plenty of water and consuming adequate fiber. While you mentioned eating fruits and vegetables, it’s important to ensure that you are getting a balance of soluble and insoluble fiber. Soluble fiber (found in oats, beans, and some fruits) can help with diarrhea, while insoluble fiber (found in whole grains and vegetables) can help with constipation.

3. Probiotics: Some studies suggest that probiotics can help restore the balance of gut bacteria and may alleviate symptoms of IBS. You might consider trying a probiotic supplement or incorporating fermented foods like yogurt, kefir, or sauerkraut into your diet.

4. Stress Management: Stress can significantly impact digestive health. Techniques such as mindfulness, yoga, or cognitive behavioral therapy (CBT) can help manage stress and may improve your symptoms.

5. Regular Exercise: Physical activity can help stimulate digestion and may alleviate symptoms of constipation. Aim for at least 30 minutes of moderate exercise most days of the week.

6. Medication: Depending on the diagnosis, your doctor may prescribe medications to help manage your symptoms. For instance, antispasmodic medications can help reduce abdominal pain, while laxatives may be prescribed for constipation.

7. Follow-Up: Since your symptoms are chronic and have not improved with initial treatments, it’s crucial to maintain open communication with your healthcare provider. Regular follow-ups can help adjust your treatment plan as needed.

In conclusion, while chronic digestive issues can be challenging, understanding the potential underlying causes and implementing a comprehensive management plan can lead to significant improvements. It’s essential to work closely with healthcare professionals to tailor a strategy that works for you. Remember, you are not alone in this, and with the right support and interventions, you can find relief from your symptoms.
