Pain

Hello Dr.
Lu,
I have noticed that my menstrual cycles have been irregular recently (11/22, 12/25, 1/13, 2/8), with an average cycle length of about 26 days.
I had a Pap smear on 1/19, and the results were normal.
I asked my doctor about the possible reasons for my irregular menstrual cycles, and they mentioned that it could be due to ovulatory bleeding, and as long as I don't experience abdominal pain, there is no need to worry; I should just monitor my upcoming cycles for regularity.
However, I have been feeling a persistent dull sensation in my left abdomen, which is not painful but continues to linger.
Additionally, I have been experiencing significant vaginal pain during intercourse.
Could this be related?
Thank you for taking the time to respond.
Thank you!

Dr. Lü Lizheng reply Obstetrics and Gynecology

Possible reasons for irregular menstrual cycles include irregular sleep patterns, staying up late, stress, obesity, and other endocrine disorders.
The persistent dull sensation in the left lower abdomen, which is not painful but continuous, may be due to uterine ligament contractions, chronic uterine inflammation, enlarged ovaries or fallopian tubes, or even intestinal contractions.
If there is severe pain during intercourse, it could be due to vaginal inflammation.
Please consult a physician for treatment.

Understanding irregular menstrual cycles and pelvic pain in women can be complex, as various factors can contribute to these symptoms. Based on your description, it seems you are experiencing irregular menstrual cycles along with persistent discomfort in your left lower abdomen and pain during intercourse. Let's break down these issues and explore potential causes and recommendations.

Irregular menstrual cycles can be influenced by a variety of factors, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions. In your case, the cycle lengths you've provided (11/22, 12/25, 1/13, 2/8) suggest some irregularity, especially since your typical cycle is around 26 days. The doctor mentioned that this could be due to ovulatory bleeding, which is not uncommon and usually not a cause for concern if accompanied by no significant pain. However, the persistent sensation in your left lower abdomen could indicate something more.

The left lower abdominal discomfort you are experiencing could be related to several conditions. One possibility is ovarian cysts, which can cause a dull ache or discomfort in the lower abdomen, especially if they rupture or become large. Another potential cause could be endometriosis, a condition where tissue similar to the lining of the uterus grows outside of it, often leading to pelvic pain, especially during menstruation or intercourse. Pelvic inflammatory disease (PID) is another consideration, which can occur due to infections and may lead to pain during sexual activity.

The pain during intercourse (dyspareunia) you mentioned is particularly concerning and can be associated with several factors, including vaginal dryness, infections, or pelvic conditions like endometriosis or fibroids. It is essential to address this symptom, as it can significantly affect your quality of life and intimate relationships.

Given these symptoms, it is crucial to follow up with a healthcare provider, preferably a gynecologist, who can perform a thorough evaluation. This may include a pelvic exam, ultrasound, or other imaging studies to assess for any abnormalities in the reproductive organs. Additionally, discussing your symptoms in detail will help the doctor understand your situation better and provide appropriate treatment options.

In the meantime, consider keeping a symptom diary to track your menstrual cycle, any pain or discomfort, and any other symptoms you may experience. This information can be invaluable for your healthcare provider in diagnosing and managing your condition.

In summary, while irregular menstrual cycles can be common and often benign, the combination of persistent abdominal discomfort and pain during intercourse warrants further investigation. It is essential to consult with a healthcare professional to rule out any serious conditions and to receive appropriate care tailored to your specific needs.
