Menstrual irregularities
I have been experiencing irregular menstrual cycles for the past couple of years.
About once every six months to a year, I have an abnormal experience; sometimes my period drags on, and other times it stops for a few days and then starts again.
Additionally, I frequently have vaginal yeast infections.
The last ultrasound and pelvic exam I had in June were normal.
The Pap smear in September was also normal, but there was a slight indication of inflammation.
I had my period on November 12, and it ended on November 17, but three days later, I started spotting again for a few days.
Given my situation and age, should I seek medical attention immediately, or can I wait and observe? Over the past couple of years, I have had ultrasounds for each abnormal occurrence.
Once, I had a polyp removed, and the bleeding stopped.
Another time, I did nothing, and it resolved itself after three months.
I am unsure why this is happening.
Is it possible that I am approaching menopause and experiencing irregularities, or could there be some malignant changes? Please help me address this concerning gynecological issue.
I have consulted a doctor, who mentioned that it might be a hormonal issue.
Can hormones really cause such repeated irregularities?

An ultrasound should normally rule out issues with uterine fibroids and the ovaries.
If menstruation is not prolonged or has stopped and then resumed, it may be related to hormonal or endometrial issues.
Additionally, the presence of an intrauterine device (IUD) can also cause abnormal bleeding.
It is recommended to visit an outpatient clinic for an endometrial biopsy, and oral progestins may be prescribed to regulate the menstrual cycle.

More Info
Irregular menstruation, or abnormal menstrual cycles, can be a source of concern for many women, especially when it becomes a recurring issue. Given your history of experiencing irregularities every six months to a year, along with other symptoms such as recurrent vaginal yeast infections and recent abnormal bleeding, it is essential to understand the potential causes and when to seek medical help.
Causes of Irregular Menstruation
1. Hormonal Imbalances: One of the most common causes of irregular menstruation is hormonal imbalance. Hormones such as estrogen and progesterone regulate the menstrual cycle, and any disruption can lead to irregularities. Factors that can cause hormonal imbalances include stress, significant weight changes, excessive exercise, and underlying health conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders.
2. Age-Related Changes: As women approach perimenopause (the transitional phase before menopause), they may experience irregular periods. This phase can begin in the 30s or 40s and is characterized by fluctuations in hormone levels, leading to changes in menstrual patterns.
3. Uterine Conditions: Conditions such as fibroids, polyps, or endometriosis can also lead to irregular bleeding. These conditions can cause the lining of the uterus to thicken or develop abnormal growths, which may result in heavy bleeding or spotting between periods.
4. Infections: Recurrent vaginal infections, such as yeast infections, can sometimes be associated with menstrual irregularities. Infections can cause inflammation and affect the menstrual cycle.
5. Medications: Certain medications, including hormonal contraceptives, can impact menstrual cycles. If you have recently started or changed any medications, this could be a contributing factor.
When to Seek Help
Given your symptoms and history, it is advisable to consult a healthcare provider if you experience:
- Significant Changes: If your menstrual cycle becomes significantly longer or shorter than your usual pattern, or if you experience heavy bleeding or prolonged periods.
- Severe Pain: If you have severe pain associated with your menstrual cycle that is not relieved by over-the-counter medications.
- Persistent Symptoms: If you continue to experience irregularities despite previous evaluations and treatments.
- Other Symptoms: If you notice other concerning symptoms such as unexplained weight loss, fatigue, or changes in appetite.
Your Situation
In your case, since you have had normal ultrasounds and Pap smears recently, it is less likely that there is a serious underlying condition. However, the fact that you are experiencing recurrent irregularities and spotting shortly after your period could indicate a need for further evaluation. Hormonal fluctuations can indeed cause these symptoms, but it is essential to rule out any other potential issues.
Recommendations
1. Monitor Your Cycle: Keep a detailed record of your menstrual cycle, including the duration, flow, and any associated symptoms. This information can be helpful for your healthcare provider.
2. Consult a Specialist: If your symptoms persist or worsen, consider seeing a gynecologist who specializes in menstrual disorders. They may recommend further testing, such as blood tests to check hormone levels or imaging studies to assess for uterine abnormalities.
3. Lifestyle Modifications: Consider lifestyle factors that may be contributing to hormonal imbalances, such as stress management techniques, maintaining a healthy weight, and regular exercise.
4. Follow-Up Care: Since you have had previous evaluations, it is essential to maintain regular follow-ups with your healthcare provider to monitor your condition and adjust any treatment plans as necessary.
In conclusion, while hormonal fluctuations can lead to irregular menstruation, it is crucial to stay vigilant and seek medical advice if your symptoms are concerning or if you feel that they are impacting your quality of life. Your health is paramount, and addressing these issues with a healthcare professional can provide you with the clarity and care you need.
