Why Is My Circumcision Site Still Swollen After a Month? - Urology

Why hasn't it improved yet?

Doctor, it's been a month since my circumcision, and the area at the back is still swollen.
I remember the doctor mentioned that it would take longer to heal due to gravity, but it's been a month and it's still swollen.
The stitches have almost all dissolved, yet the swelling persists.
During my last follow-up appointment, the doctor said there was no issue, but could it be possible that some gauze was left inside? I'm really concerned that the swelling hasn't improved after a month.

Hello! Please go for a re-evaluation with the surgeon, or consider getting a second opinion from another doctor, as discussing the issue solely via email will not lead to a resolution.
Sincerely, Dr.

It’s understandable to be concerned about persistent swelling at the circumcision site, especially after a month has passed since the procedure. While I cannot provide a specific diagnosis without a physical examination, I can offer some insights into what might be happening and what you should consider.

Swelling after circumcision is not uncommon, and it can occur for several reasons. Initially, the surgical site may swell due to the body's natural inflammatory response to surgery. This inflammation is part of the healing process, as the body sends blood and immune cells to the area to promote healing. However, if the swelling persists beyond a few weeks, it may indicate other issues.

1. Normal Healing Process: In many cases, swelling can take time to resolve. The area may remain swollen for several weeks, especially if there was significant tissue manipulation during the procedure. Factors such as individual healing rates, the extent of the surgery, and even the patient's overall health can influence recovery time.

2. Fluid Accumulation: Sometimes, fluid can accumulate in the surgical area, leading to a condition known as a seroma. This is a pocket of clear fluid that can form after surgery and may cause swelling. If this is the case, it may resolve on its own, but in some instances, it might require drainage by a healthcare professional.

3. Infection: Another possibility is an infection at the surgical site. Signs of infection can include increased redness, warmth, pain, and discharge from the incision site. If you notice any of these symptoms, it’s crucial to contact your healthcare provider promptly, as infections may require antibiotics or further intervention.

4. Suture Reaction: If dissolvable sutures were used, it’s possible that the body is reacting to the sutures, which can sometimes cause localized swelling. If the sutures are not dissolving as expected, or if there is a reaction, this could contribute to prolonged swelling.

5. Underlying Conditions: In some cases, underlying conditions such as lymphatic issues or other medical concerns can affect healing and cause persistent swelling. If you have any pre-existing conditions that might affect healing, it’s worth discussing these with your doctor.

6. Concerns About Foreign Bodies: Your concern about the possibility of gauze or other materials being left inside is valid. While this is rare, if there is any suspicion that foreign material may have been left behind, it is essential to follow up with your healthcare provider. They may perform an examination or imaging studies to ensure that everything is as it should be.

Given that your doctor has indicated that everything appears normal, it may be helpful to seek a second opinion if you continue to feel uneasy about the swelling. It’s essential to advocate for your health and ensure that your concerns are addressed.
In summary, while some swelling can be a normal part of the healing process after circumcision, persistent swelling after a month warrants further evaluation. Keep monitoring the site for any changes, and don’t hesitate to reach out to your healthcare provider with any new symptoms or concerns. Communication with your doctor is key to ensuring a smooth recovery.
