Calf muscle soreness?

Hello Doctor: My mother is 54 years old and has been experiencing muscle soreness in her calves for the past month or two.
She walks normally, but experiences significant pain when going up and down stairs or squatting.
There is no joint pain.
What could be the issue? Should she see an orthopedic specialist or another type of doctor? Thank you for your response.

Dr. Ye Qingnian reply Orthopedics

Experiencing pain while going up and down stairs, especially when descending, is more pronounced in the knees as they bear greater weight.
For a 54-year-old woman, the most likely conditions are osteoarthritis or chondromalacia patellae.
It is advisable to consult an orthopedic clinic for evaluation.

Calf muscle pain can be a common issue, especially in individuals who may be more active or have underlying health conditions. In your mother's case, the symptoms of calf muscle soreness during specific activities like going up and down stairs or squatting, while not experiencing pain during regular walking, can suggest a few potential causes.

One of the most common causes of calf muscle pain is muscle strain or overuse. This can occur if the muscles are not accustomed to certain activities, such as climbing stairs or squatting, which require more strength and flexibility. Additionally, age-related changes in muscle elasticity and strength can contribute to discomfort during these activities.
Another possibility is that your mother may be experiencing symptoms related to tendonitis, which is inflammation of the tendons in the calf area. This condition can arise from repetitive movements or overexertion, leading to pain during specific motions.
Additionally, conditions such as Achilles tendonitis or even issues related to the gastrocnemius muscle (the larger calf muscle) could be at play. These conditions often manifest as pain during activities that put stress on the calf muscles, such as stair climbing or squatting.

It is also important to consider the possibility of underlying conditions such as arthritis, which can affect the joints and lead to discomfort during certain movements, even if the joints themselves do not feel painful at rest.
Given that your mother is experiencing pain specifically during certain movements, it would be advisable to start with a visit to a primary care physician or a specialist in orthopedics. They can perform a thorough examination, which may include assessing her range of motion, strength, and any potential swelling or tenderness in the calf area. They may also recommend imaging studies, such as an ultrasound or MRI, if they suspect any underlying tendon or muscle injuries.

If the orthopedic evaluation does not reveal any significant issues, a referral to a physical therapist may be beneficial. Physical therapy can help strengthen the calf muscles, improve flexibility, and provide strategies to manage pain during activities. A physical therapist can also guide your mother through specific exercises tailored to her needs, which can help alleviate pain and prevent future injuries.

In summary, calf muscle pain during specific activities can stem from various causes, including muscle strain, tendonitis, or age-related changes. It is essential to seek a professional evaluation to determine the underlying issue and develop an appropriate treatment plan. Starting with an orthopedic specialist is a good approach, and they can guide further management, including physical therapy if necessary.
