White filamentous attachments at the hair roots?
Hello, doctor.
Most of the hair roots on my scalp have a white, thread-like attachment covering them.
It resembles small segments, varying in length but generally less than 0.2 centimeters, and is as fine as the hair itself.
This has been present for many years, and I have been unsure about what this white thread-like attachment is.
(P.S.
I also have dandruff.) I hope you can help me with this.
Thank you!

Dr. Jiang Zheen reply Dermatology
If the white attachment can be pulled off, it is a type of circular dandruff and can be treated like regular dandruff.
Wishing you safety.

More Info
The white thread-like structures you are observing at the roots of your hair could be indicative of several conditions, but they are most likely related to either sebum buildup or a form of dandruff. Given that you also mentioned having dandruff, it is possible that these white structures are a manifestation of your scalp condition.
Sebum is an oily substance produced by sebaceous glands in the skin, including the scalp. When there is an overproduction of sebum, it can mix with dead skin cells and other debris, leading to the formation of white, thread-like structures that can cling to the hair follicles. This is often seen in individuals with oily scalps or those who may not wash their hair frequently enough to remove excess oil and buildup.
Additionally, the presence of dandruff, which is characterized by flaking and itching of the scalp, can also contribute to the appearance of these white structures. Dandruff is often caused by a yeast-like fungus called Malassezia, which thrives on the scalp and can lead to increased cell turnover, resulting in flakes. These flakes can sometimes appear as white, thread-like structures when they adhere to the hair.
If these white structures can be easily removed, they may simply be a form of dandruff or scalp buildup that can be managed with proper hair care. Regular washing with a gentle shampoo, possibly one formulated for dandruff, can help reduce the buildup and improve the overall health of your scalp. It may also be beneficial to incorporate a clarifying shampoo occasionally to remove excess oil and product buildup.
However, if the white structures are persistent, accompanied by significant itching, redness, or irritation of the scalp, it would be advisable to consult a dermatologist. They can provide a thorough examination and determine if there is an underlying condition such as seborrheic dermatitis, psoriasis, or even a fungal infection that may require specific treatment.
In summary, the white thread-like structures at your hair roots are likely related to sebum buildup or dandruff. Maintaining a proper hair care routine and consulting with a dermatologist if symptoms persist will help in managing the condition effectively.
