Nipple pain

Hello, doctor.
I have noticed small pimples on and around my areolas on both sides.
Is this normal? Recently, I felt a vague pain in my left nipple, and there was white discharge.
When I squeezed it, a white substance resembling a cyst and large pimples mixed with blood-tinged fluid came out.
Should I be concerned about any changes in my breast?

Keiko, 20~29 year old female. Ask Date: 2005/05/09

Dr. Li Guanghan reply Obstetrics and Gynecology

Nipple pain and associated symptoms can be concerning, and it's important to understand the potential causes and when to seek medical help. Based on your description, you are experiencing nipple pain, the presence of small bumps around the areola, and white discharge that resembles pus mixed with blood. Let's break down these symptoms and their possible implications.

Causes of Nipple Pain
1. Hormonal Changes: Hormonal fluctuations, particularly during menstrual cycles, pregnancy, or breastfeeding, can lead to nipple sensitivity and pain. The hormonal changes can also cause the development of small bumps on the areola, which are often normal and can be attributed to Montgomery glands—sebaceous glands that lubricate the nipple.

2. Infections: Conditions such as mastitis (an infection of breast tissue) can cause pain, swelling, and discharge from the nipple. This is more common in breastfeeding women but can occur in others as well. The presence of pus-like discharge, especially if accompanied by redness or warmth, may indicate an infection.

3. Duct Ectasia: This condition occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens. It can lead to pain, discharge, and sometimes a lump near the nipple.

4. Benign Growths: Conditions such as fibrocystic changes or benign tumors can also cause pain and discharge. These are usually not serious but should be evaluated by a healthcare professional.

5. Paget's Disease of the Nipple: This is a rare form of breast cancer that can cause changes in the nipple and areola, including pain, itching, and discharge. It is essential to rule this out, especially if symptoms persist or worsen.

Symptoms to Monitor
- Pain: Is the pain constant, or does it come and go? Is it sharp, dull, or throbbing?
- Discharge: The nature of the discharge is crucial. Is it clear, bloody, or pus-like? The presence of blood is particularly concerning.

- Changes in Appearance: Look for any changes in the skin of the nipple or areola, such as redness, scaling, or a change in texture.

- Lumps: Any new lumps in the breast or underarm area should be evaluated.

When to Seek Help
Given your symptoms, particularly the presence of blood in the discharge and the pain, it is advisable to seek medical attention promptly. A healthcare provider will likely perform a physical examination and may recommend imaging studies, such as a mammogram or ultrasound, to assess the underlying cause of your symptoms.

Conclusion
While some changes around the nipple can be normal, the combination of pain, discharge, and the appearance of bumps warrants further investigation. It is essential to consult with a healthcare professional who can provide a thorough evaluation and appropriate management. Early diagnosis and treatment are crucial, especially if there is a risk of infection or other underlying conditions. Remember, it is always better to err on the side of caution when it comes to breast health.
