Chest rib pain

Hello doctor, I recently noticed that when I sleep on my right side, I experience pain in the lower part of my rib cage.
Changing positions while sleeping alleviates the pain.
However, pressing on the area doesn't cause pain, but there is a slight discomfort when I press on the epigastric region.
I also feel a mild pain in the area corresponding to the right side of my heart when I take a deep breath.
I'm wondering if this could be due to an injury from playing basketball.
What might be the issue, and which medical specialty should I consult? Thank you.

Dr. Shi Guozheng reply Orthopedics

Chest bone pain may be caused by excessive strain during exercise, and this phenomenon typically resolves within three to five days.
If the pain does not improve, it is necessary to consult an orthopedic physician or consider getting an X-ray to check for any issues with the ribs.

More Info

Rib pain can arise from various causes, and understanding these can help you determine when to seek medical assistance. Given your symptoms, it’s essential to consider both musculoskeletal and potential internal causes.

Possible Causes of Rib Pain
1. Musculoskeletal Issues: The most common cause of rib pain, especially if it occurs during movement or certain positions, is musculoskeletal strain. This can happen due to activities like playing basketball, where twisting or sudden movements may strain the muscles or ligaments around the ribs. Conditions such as costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can also lead to localized pain.

2. Injury: If you suspect that your rib pain may be related to a recent activity, such as playing basketball, it could be due to a rib injury or a muscle strain. Even if you do not recall a specific incident, repetitive motions or awkward movements can lead to pain.

3. Internal Organ Issues: Pain that is felt in the rib area can sometimes be referred pain from internal organs. For instance, issues with the lungs (like pleurisy or pneumonia), heart (like angina), or even gastrointestinal problems (like acid reflux or gallbladder issues) can manifest as rib pain. The discomfort you feel when pressing on your epigastric area (the area just below the ribs) could suggest a gastrointestinal issue.

4. Nerve-related Pain: Conditions like intercostal neuralgia, where the nerves running between the ribs become irritated, can also cause pain that may be exacerbated by movement or certain positions.

When to Seek Medical Help
You should consider seeking medical attention if:
- Persistent Pain: If the pain continues or worsens over a few days, it’s advisable to consult a healthcare provider.

- Severe Pain: If you experience severe pain that interferes with your daily activities or sleep, this warrants a medical evaluation.

- Associated Symptoms: If you develop additional symptoms such as difficulty breathing, chest tightness, fever, or any gastrointestinal symptoms (like nausea or vomiting), you should seek immediate medical attention.

- History of Trauma: If you have had a recent fall or injury, it’s important to rule out fractures or more serious injuries.

Which Specialist to See
Given your symptoms, starting with a visit to a primary care physician would be a good first step. They can perform an initial evaluation and may refer you to a specialist if necessary. Depending on the findings, you might be referred to:
- Orthopedic Specialist: If the pain is determined to be musculoskeletal in nature.

- Pulmonologist: If there are concerns about lung-related issues.

- Cardiologist: If there are signs that suggest heart-related problems.

- Gastroenterologist: If gastrointestinal issues are suspected.

Conclusion
In summary, rib pain can stem from various causes, ranging from musculoskeletal strains to potential internal organ issues. Given your description of the pain being positional and associated with certain movements, it may be related to a muscle strain or costochondritis. However, due to the complexity of rib pain and the potential for serious underlying conditions, it’s crucial to monitor your symptoms closely and seek medical advice if they persist or worsen. Early intervention can lead to more effective management and treatment of your symptoms.
