Am I Pregnant? Menstrual Delays and Ovarian Cysts - Obstetrics and Gynecology

I'm not sure if I'm pregnant..?

Hello, Doctor.
My last menstrual period started on the 18th of last month.
I usually have a delay of 4 days, but last month it was delayed by 5 days.
This month, I expected my period to arrive around the 18th or 19th, but it hasn't come yet, and I don't feel like it's coming.
I had sexual intercourse during this period, but we used a condom.
I later went to the hospital and found out I have a 5 cm cyst.
The doctor prescribed medication to induce menstruation for 3 days.
Today is the third day, and I still don't feel like my period is coming.
On June 22, I bought a pregnancy test and it came back negative.

1.
Is my missed period due to the cyst?
2.
Am I likely not pregnant?
3.
After taking the medication, how many days should I wait for my period to come? If it still doesn't come, should I go back for another check-up?
4.
If the cyst doesn't go away, are there other options besides surgery? I get very anxious whenever my period is late.

Xiao Miao, 20~29 year old female. Ask Date: 2005/06/23

1.
Ovarian cysts and menstrual irregularities may be related, but the correlation is not significant.
2.
To confirm pregnancy, please visit a gynecologist.
3.
After taking menstrual induction medication, menstruation will typically occur about three to five days later.
4.
For ovarian cysts, blood tests should be conducted first to rule out the possibility of malignancy, followed by ultrasound monitoring of the cyst's changes, which may include enlargement, no change, or disappearance.
Laparoscopic surgery is a good option.

It sounds like you're experiencing a complex situation regarding your menstrual cycle, potential pregnancy, and the presence of an ovarian cyst. Let's break down your concerns and provide some clarity.

1. Understanding Menstrual Delays: Menstrual cycles can be influenced by various factors, including hormonal imbalances, stress, changes in weight, and underlying medical conditions such as ovarian cysts. In your case, you mentioned having a 5 cm cyst, which could potentially affect your menstrual cycle. Ovarian cysts can sometimes lead to irregularities in menstruation, including delays or missed periods. However, it's important to note that not all cysts will cause such symptoms, and many women with cysts have regular cycles.

2. Pregnancy Concerns: You indicated that you have had sexual intercourse while using protection, which significantly reduces the likelihood of pregnancy. Additionally, you performed a home pregnancy test that returned negative. Home pregnancy tests are generally quite accurate, especially when taken after a missed period. Given that you have a negative test and used protection, it is reasonable to conclude that you are not pregnant. However, if you continue to experience symptoms or have concerns, it may be beneficial to consult with a healthcare provider for further evaluation.

3. Effects of Medication: You mentioned taking medication to induce your period. The time it takes for your menstrual cycle to normalize after taking such medication can vary. Typically, if the medication is effective, you may expect your period to start within a few days after completing the course. However, if your period does not arrive within a week after finishing the medication, it would be advisable to follow up with your healthcare provider for further assessment.

4. Management of Ovarian Cysts: As for your ovarian cyst, the management depends on various factors, including the size of the cyst, symptoms, and whether it is functional or pathological. Many cysts resolve on their own without intervention. If the cyst persists or causes significant discomfort, surgical options may be considered. However, there are also non-surgical approaches, such as monitoring and hormonal treatments, that can be effective. It's essential to have a thorough discussion with your healthcare provider about the best course of action based on your specific situation.

In summary, while your menstrual delay may be related to the ovarian cyst, the negative pregnancy test suggests that pregnancy is unlikely. If your period does not resume after taking the medication, or if you have ongoing concerns about the cyst, it is crucial to seek further medical advice. Regular follow-ups with your healthcare provider can help ensure that any underlying issues are addressed and that you receive appropriate care tailored to your needs. Remember, it's always better to err on the side of caution when it comes to your health.
